Social Prescribing Day, 26th March

You may have heard the term ‘social prescribing’ and wondered what it is, and can it help me?

Social prescribing is a service to help boost wellbeing by linking you into local support networks or services, as part of an overall approach to individual care.

Social Prescribers are link workers that have access to a wide range of local agencies, best suited to help with your overall health and wellbeing. In this PCN you might see a Social Prescriber for the following reasons:

Helping to access services from social care, pharmacies, allied health professionals, fire service, housing associations, job centres, employment and/or volunteering support
Needing help with debt, benefits and housing, loneliness, accessing social groups, mental health and support groups.

To request an appointment with our Social Prescribing Team, submit an online consultation request or contact the surgery reception to ask for an appointment, stating the reason why you would like to be seen and the best telephone number to call you on and one of the Social Prescribers will arrange an appointment.
