Adult Social Care and Health services in Aberdeenshire are coordinated and delivered by the Aberdeenshire Health and Social Care Partnership (AHSCP), a partnership of Aberdeenshire Council and NHS Grampian. The AHSCP provides a range of care and support services in the community for people with different care needs, mainstream services, which are available to all, and specialist services designed

for more complex needs. AHSCP services take a coordinated approach to the way care, support and lifestyle advice designed to keep you healthier and independent for longer is delivered. AHSCP has identified the following health improvement priorities to ensure our communities are healthy places to live, those living in our communities are able to stay resilient and are well connected to a wide range of services, support and activities in their communities:

• Improving mental health and wellbeing.
• Healthy eating, being active and healthy weight.
• Minimising the impact of poverty and inequalities.
• Building resilience and community capacity for health improvement. These priorities will be progressed in collaboration with our communities and partners across health and social care, local government and the third sector. The Aberdeenshire Health and Social Care Partnership Public Health Team has a key role in supporting the delivery of these priorities partners to deliver this plan through:

• Gathering and interpreting evidence and health information and data.
• Development of policies and strategies.
• Development of delivery plans and health improvement programmes.
• Assessing the impact of plans and programmes.
• Shifting service provision towards prevention and early intervention.

💛Pillar Kincardine will be out in the community this World Su***de Prevention Day, bringing messages of hope, connection and support to Stonehaven. Please stop by, say hello and help fill the Train of Hope with messages that remind people that no one has to face things alone.💛

World Su***de Prevention Day, Thursday 10th September we’re taking our message of hope, connection and support out into the community. We’ll be taking over the Stonehaven Land Train and parking at 3 locations around town (see Poster for times and locations), creating a welcoming space where anyone can stop by for a chat, find out more about Pillar’s support services, or simply say hello. We’ll also be creating a special display with messages of hope and remembrance. You can leave a message for someone you care about, remember someone you’ve lost, or simply share a few words that might bring hope to someone who needs it. Your words could make a real difference. If you can’t find the land train on its travels, we’ll also have a stall at the community garden in the Market Square (opposite Greggs).
Come along, say hello and help us fill our Train of Hope with messages that remind our community: there is always hope, and no one has to face things alone.
