Heartwood Medical Practice We are a GP surgery based in Swadlincote, based at Swadlincote Health Centre on Civic Way, DE11 0AE.

Out team includes GPs, Advanced Nurse Practitioners, Practices Nurses, HCAs, Health Coach and a Social Prescriber, all supported by our admin team. We are a team of 6 GP's, 2 Advanced Nurse Practitioners, 4 Practice Nurses, a Practice Pharmacist, a Health and Well-being Worker, Social Prescriber and 2 Health Care Assistants, as well as a reception and admin team. Please see the attached posters, one is for the patients and one for the carers, regarding a study on how people with ongoing musculoskeletal pain use pain-relieving medicines. The information from the study will help create a new way to support people in managing their condition more effectively.

UPDATE 17.25 - we have diverted our phone lines to mobile phones so can now accept incoming calls.

Unfortunately our phone lines are down. We have a technician trying to resolve for us and hope to be back up and running soon.

This year the flu vaccine programme will be delivered in Practice. We will be starting to contact eligible patients within the next few weeks. Pregnant women and children will be eligible from September and those eligible have already been contacted, except school age children with no risk factors, whose vaccines will be delivered by the school health team. From 1st October the remaining risk groups will be eligible, so those aged over 65, those aged 18-64 in a clinical risk group, those in long stay care homes, carers and close contacts with immunocompromised individuals.

An amber heat-health alert is in place across parts of England from today until 14 August.

During an amber alert, hot weather can put more people’s health at risk – not just those who are usually more vulnerable.

Older people, young children and people with existing health conditions may be particularly at risk.

Take extra care and look out for others who may struggle in the heat.
