LOWER BACK PAIN? I DONโ€™T ALWAYS START WITH THE LOWER BACK. ๐Ÿ‘‡

When someone presents with lower-back pain, it can be useful to look at how other areas are moving too: including the hips and surrounding muscles.

Here Iโ€™m using Muscle Energy Technique (MET) with the TFL, a muscle around the front/side of the hip.

MET involves gently contracting a muscle against resistance, followed by relaxation and movement.

If assessment identifies tightness or restricted movement around the hip, this may be one of several areas considered as part of the wider treatment approach.

But that doesn't mean a โ€œtight TFLโ€ is automatically the cause of someone's lower-back pain.

That's why I prefer to:

ASSESS โ†’ TREAT โ†’ REASSESS

Understand what I'm finding first, choose a technique for a reason, and then see how the person responds.

That's my treatment ethos: treating with purpose and intent rather than simply working where it hurts.

Great catching up with Ben from Boro Runners recently and talking all things sports massage, soft tissue therapy and the realities of building a business in this industry.

As I continue building Syd Ha Performance & Soft Tissue Clinic, I'm keen to learn not only through my clinical training, but also from people who have already gained experience working with clients and building their own practice.

We had a really useful conversation around building a client base, running a treatment business and some of the lessons Ben has picked up along the way.

Plenty for me to take away and put into practice.

LOWER BACK FEELING TIGHT OR STIFF? ๐Ÿ‘‡
Here Iโ€™m working on the QL, a muscle found deep in the lower back.

Iโ€™m using a technique called Soft Tissue Release (STR), which combines targeted pressure with movement to work on the muscle.

The QL is one of several muscles I may consider when someone comes to me with lower-back pain, tightness or restricted movement.

But lower-back pain can have many different factors involved: which is why I don't automatically treat the same area in everyone.

First, I assess.
Then I treat based on what I find.
Then I reassess.

And that's what my treatment ethos is about: treating with purpose and intent, rather than simply working on an area because it feels tight.

Every technique should have a reason behind it.

When someone presents with lower-back pain, the assessment may also involve looking at the hips and surrounding structures.

Here Iโ€™m performing a Modified Thomas Test during my Level 5 Clinical Sports & Remedial Massage training.

This test can form part of an assessment of hip flexor length and hip movement, helping to build a wider picture of what may be relevant to someoneโ€™s presentation.

Lower-back pain isnโ€™t always about finding one โ€œtight muscle.โ€

I want to understand:

- What movements affect your symptoms?
- Are there relevant restrictions or differences?
- What is the pain stopping you from doing?

Then treatment can be based on what the assessment actually finds.

LOWER BACK PAIN AFFECTING YOUR DAY?

Sometimes the biggest problem isn't just the pain itself.

It's what the pain starts stopping you from doing.

Maybe you're struggling to:

โ€ข Sit comfortably at work
โ€ข Bend down or put your shoes on
โ€ข Lift and carry things
โ€ข Exercise or play sport
โ€ข Sleep comfortably
โ€ข Get through normal day-to-day activities

That's why when someone comes to me with lower back pain, I'm interested in two things:

1: What are you feeling?
2: What is it stopping you from doing?

Treatment isn't simply about working on the area that hurts.

I use an assessment-led approach to understand your symptoms, movement and function before tailoring hands-on treatment to your presentation.

Then we reassess.

PAIN MATTERS. FUNCTION MATTERS TOO.

FEELING STRESSED & CARRYING TENSION? ๐Ÿ˜ฎโ€๐Ÿ’จ

You don't have to be injured or in significant pain to benefit from hands-on treatment.

Stress can sometimes go hand-in-hand with muscle tension and feelings of tightness, particularly around the neck, shoulders and upper back.

Sometimes what you need is an opportunity to slow down, relax and recover.

And recovery isn't only important after an injury.

Taking time to recover can support how you feel, move and perform: whether that's:

โ€ข At work
โ€ข In the gym
โ€ข Playing sport
โ€ข Or simply getting through everyday life feeling better

Treatment can be tailored to how you're presenting and what you want from the session.

Because looking after your body isn't only about dealing with pain when something goes wrong.

LOWER BACK PAIN? LET'S TALK ABOUT THE ERECTOR SPINAE ๐Ÿ‘‡

The erector spinae are a group of muscles running along the spine that play an important role in supporting the spine, maintaining posture and controlling movement.

When someone presents with lower back pain, stiffness or restricted movement, these muscles may be one of several areas considered during assessment and treatment.

- In this training video, I'm working on the erector spinae with a fellow student as part of my ongoing Level 5 Clinical Sports & Remedial Massage training.

But importantly: lower back pain isn't always about one muscle.

That's why I don't simply find where it hurts and start treating.

I look at the person's symptoms, movement and relevant assessment findings before deciding what may be appropriate to work on.

Because pain matters: but so does getting you back to moving and functioning better.

Pain is often what makes someone seek help.

But there's another important question:

What is the pain stopping you from doing?

Maybe it's affecting your ability to:

โ€ข Work comfortably
โ€ข Walk or move normally
โ€ข Train or play sport
โ€ข Sleep comfortably
โ€ข Bend, lift or carry
โ€ข Keep up with everyday activities

That's where function matters.

For me, treatment isn't only about asking:

โ€œHow much does it hurt?โ€

I also want to know:

โ€œWhat are you struggling to do because of it?โ€

Because reducing symptoms is important: but so is helping you get back to the work, sport and everyday activities that matter to you.

PAIN + FUNCTION = THE WIDER PICTURE

That's why my approach is:
