TREATMENTS - OVERVIEW
The Foot Doctor Treats all Foot Ankle and Leg Pain. As a Telford based Sports and MSK Podiatrist, I treat all lower limb injuries, From ingrown toenails to Runners knee. Irrespective of activity, age or condition, I will aim to get you moving again! Treatments Available

Treatment is as wide ranging as the actual injuries and medical problems that are dealt with.

• Self Help - A definite and simple set instructions will be issued to you. This may include basic activity modification, strengthening and footwear advice. Or more comprehensive assistance or prescription medication.

• Strength/Flexibility withPhysio and Taping - Basic exercises that take little time will be provided. If you are an active sports person or athlete, a full program and review structure for solving muscle imbalance and injury will be provided.

• Orthotics - A 3D laser scan allows accurate casting and the finest orthotics to be personally designed and manufactured by myself, in my own international orthotic insole laboratory based here in Telford. We have a dedicated orthotic page for a reason, when it comes to foot and ankle pain, they are clinically proven to stop pain and reverse injury. But only when used properly and only when they are of the finest quality. This is not possible elsewhere, only two other such facilities exist within the Great Britain.

• Shockwave Therapy - A large investment in Orthotics and now Shockwave therapy, has been made in our Telford and Shrewsbury based pain clinic. £100, 000 of equipment has been self-funded for a reason; because it works. Shockwave therapy stops pain, rapidly speeds healing and rehabilitates otherwise chronic injured that would require surgery to otherwise resolve.

• Steroid Injections - A useful adjunct in acute and very painful cases, aiding rehabilitation. Not a cure, but prevents excessive pain relief causing Stomach problems and is far more effective. Rarely used or indeed required.

• Podiatry and Chiropody - I have availability for the usual Nail Complaints, fungal infections and hard skin that can be treated with great quality Chiropody and Podiatry here in Telford.

• Surgery - Soft tissue surgery with anaesthetic to treat great Verrucae, warts and Ingrown nails. Relatively painless and effective.

• Antibiotics - Both infected nails and skin infections are quite common!

• Clearanail for Fungal Nails - A clinically effective, easy and painless solution for your discoloured nails. I will often tell patients that "If I cannot stop your pain or fix your injury - it is unlikely to be fixed". This is testament to the broad scope of diagnostic skill, equipment and myriad of various treatment modalities that are offered for foot, ankle and leg pain here in my Telford and Shrewsbury Office.

I can say first hand, that the clinical effects of this phenomenon are encountered regularly.

Very interesting and important for clinicians to understand.

To those EDS patients receiving additional L.A this month - you are welcome ✅😁❤️

Did you know that a large proportion of people with Joint Hypermobility Syndrome (JHS) and Ehlers Danlos – Hypermobility Type appear to be resistant to local anaesthetics, either as topical creams or injections?

In one large survey JHS patients were 3 times more likely to report the poor effectiveness of local anaesthetic compared to people without JHS.

Plantar plate injuries, capsulitis and ‘metatarsalgia’ are oft treated in the same step wise fashion.

Kevin Kirby here demonstrates a typical acute swelling associated with these injuries, chronic variations will not see the swelling.

Remember to call for help, earlier, not later.

Heavily supinate feet and Peroneal Tendon injuries go together…..

A great illustration, again, from Kevin Kirby, explaining this issue.

Probably the most common reason for me issuing custom orthotics here in Telford.

Another great overview by Kevin Kirby.

Another great overview by Kevin Kirby.

What is the Plantar Plate?

The plantar plate is a fibrocartilaginous structure, approximately 11 mm wide from medial to lateral, 19 mm long from proximal to distal, and about 2-5 mm thick in an adult foot. It lies in direct contact with the plantar aspect of the metatarsal head, with the flexor digitorum longus and brevis tendons being directly plantar to it.

The plantar plate attaches distally to the proximal phalanx base and proximally to the plantar fascia. As such, the plantar plate should be considered as a distal mechanical extension of the plantar fascia, producing significant plantarflexion moment to the proximal phalanx which is absolutely necessary for digital stability and to maintain normal digital purchase during weightbearing activities.

Therefore, significant plantar plate tears can cause plantar metatarsophalangeal joint swelling, pain, a floating digit and, over time, a hammertoe deformity. The following article gives an excellent overview of the anatomy and biomechanics of the plantar plate.

Long Covid, in certain groups, appears to improve with exercise and activity.

This is also my personal finding, a point I discuss with patients weekly. To each of us, we travel our own path. But as I say to others suffering, it is worth considering.

This is a contrast to long held beliefs.

Researchers say a six-week exercise programme helped patients make "significant" improvement.
