Nutritional therapy - what exactly is it?

Hint: it is NOT an off the shelf "diet" sheet.

I recently went out for a meal with friends - and met some friends of friends, who I hadn't spoken to before. Thus we chatted and I was asked the inevitable question - "what do you do?"

I explained that I am a qualified nutritional therapist, but it's only my "side hustle" as in this economic climate, people are feeling the pinch. (Not to mention, I suck at marketing myself...)

After I had told him this, he replied: "Ok. So what *is* a nutritional therapist? What exactly do you do?"
He wasn't being unkind or awkward, genuinely interested.
Huh. I thought to myself. I wonder if that is a question that others ask as well?

So, here is a synopsis of my training, and my legal scope when dealing with clients:

TL/DR: Nutritional therapists are there for the people who are feeling "not quite right, but my blood tests are normal...", or for anyone who needs to know more about managing a specific disorder, disease, or condition. We will work with you, and alongisde any other health care providers you have, to establish the best way forward nutritionally, pharmaceutically, supplementally and with life style changes. If I can't help, I will do my best to find you a NT who I trust professionally, who has a greater specialism in that area of health.

Over three years, I learned about the entire body, from the names of toe bones through to muscles, the brain, cells, blood, organs, and commonest illnesses/conditions to affect each of the systems. I can help you manage a diagnosed/existing condition with advice on lifestyle changes, the foods that would benefit you the most, and where necessary, supplementation. We have been trained to understand the interactions between the commonest pharmaceutical drugs and supplements [mineral, vitamins, herbal] and can make recommendations for supplemental inclusions, and what supplements to avoid, if necessary.

TRAINING:

Biomedicine: I spent the first year studying the body and systems. These covered: musculo-skeletal; the heart, and cardiovascular network [veins, arteries, capillaries etc.]; respiratory; digestive; endocrine; skin and lymphatic system; urinary; reproductive; nervous system; immune; infectious diseases; oncology; sense; pharmacology; and common laboratory test results.

Year 2 involved some deeper dives into the above systems but from a nutritional stand point; biochemistry; philosophies of healing such as traditional Chinese medicine, energetics, and Ayurvedic; minerals, vitamins, supplements, and understanding research methods and scientific papers.

Year 3 was essentially learning about the commonest diseases and conditions that affect the bodily systems; proteins; fats; carbohydrates; the impact of genetics on our ability to properly digest and absorb the necessary nutrition from our diets; stress, fatigue, obesity; sports nutrition; fertility and pregnancy; and naturopathic techniques such as mindfulness, forest bathing, restorative movement.

SCOPE OF PRACTICE

As you can see, I have covered the entire body so in theory, I could help anybody with anything, with these caveats:

1. Legally I am not qualified to diagnose conditions (it's above my pay grade), nor can I prescribe medicines. For this you need a Registered Dietician.
2. I cannot work with people who have eating disorders. That is a hard professional line that we must not cross unless we have additional qualificalications in psychology or psychiatry.
3. Neither are we allowed to claim "cures".

My area of specialism is conditions of the endocrine system - the big hitters are cortisol, insulin, oestrogen, progesterone, testosterone, and thyroid. Between these they have a HUGE impact on one's metabolic health, mental health/mood, digestion & sleep, and also I was personally held hostage by my endocrine system for my entire adult lifetime, so I had a vested interest in learning more about it.

Costs?

My charges start at £100.

£100 would pay for a generic, pdf information package into any ONE particular condition that you wish to learn more about. These can be unrelated to hormones, as they are strongly research based.
The information would tell you how it manifests in the body; the common pharmacological routes to reduce the symptoms; GENERIC dietary, supplemental, and life style changes you can make to minimise its impact on the sufferer, and dangerous interactions with the commonest medicines prescribed for the condition. It would also advise about the common comorbidities.

For personalised nutritional therapy for hormonal issues which are by their nature, a slow thing to turn around, a twelve week course is the minimum commitment. I will construct 12 weeks, six months or annual packages tailored specifically to you. Prices will vary.

An example: £550. The Endocrine Foundation Program: twelve weeks.

Package Inclusions & Boundaries

Consultation Time: 1 x 75-min Initial + 2 x 45-min Follow-ups (3 sessions total).

Asynchronous Support: Portal messaging for quick clarifications (e.g., supplement questions or food swaps). Answered Tuesdays and Thursdays between 10 AM and 2 PM only.

Test Interpretation: Up to 2 lab reports reviewed (e.g., GP blood panels, DUTCH test, or private thyroid panels). Any costs of laboratory tests are in addition to my fees.

Random but vitally important public service announcement:

I have just been doing some research for something I am writing, and found various scientific articles pertaining to an issue that not everyone will be aware of - although I am very aware of it, as I suffered from it myself.

Question one: are you aware of a condition called PMDD? [Pre-menstrual dysphoric disorder]
It is a serious form of cyclical depression, related to a woman's menstrual cycle. It is like PMS [pre-menstrual syndrome] 'on steroids', and has an incredibly debilitating affect on a woman's ability to function normally in her day-to-day life for that time in her cycle when estrogen is lowest, and can lead to severe mental health problems, su***de ideation, and self-harming.

Question two: do you have a female friend or family member or partner, who is medically diagnosed ADHD ?

Question three: did you know that the percentage of women assigned female at birth who have been diagnosed with ADHD, who ALSO have PMDD, is 45% - nearly one in two. (source: doi: 10.1007/s00737-011-0232-4 )

For perspective: the incidence of PMDD in American women was between 3-8% according to a study taken in 2003. (source: doi: 10.1016/s0306-4530(03)00098-2 )

Quite the difference, yes? from less than 8/100 to nearly 1/2

So: your ADHD female friend/partner/relative is statistically at risk from suffering a severe form of cyclical depression, related to her menstrual hormones. Look out for symptoms of worsening depression, especially if she is using a form of contraceptive that does not contain estrogen, as estrogen is protective of both dopamine and serotonin levels. This is why the condition worsens during the menstrual cycle when those levels are no longer protected.

And: If you are a female, suffering from a severe form of depression that seems to get dramatically worse during your cycle, talk to your doctor about PMDD.
