Why Is My Annual Review So Important? | Holes Lane Medical Centre

Annual reviews are an essential part of managing long-term health conditions such as diabetes, asthma, COPD, high blood pressure, heart disease and other chronic illnesses. They allow us to:

Check how well your condition is being managed.
Review your medications and make any necessary changes.
Carry out important health checks and monitoring.
Help prevent complications and identify any concerns early.
Answer any questions you may have about your health.

Even if you're feeling well, attending your annual review helps us ensure you're receiving the best possible care and keeps your health records up to date.

Nationwide are partnered with Dementia UK, a specialist dementia nursing charity. It's specialist Nurses known as Admiral Nurses offer face to face clinics in Nationwide branches.
Appointments are held in a private space and you don't have to be a Nationwide member to access the service. ❤️

‼️Due to an unexpected outage affecting our internet systems, we are currently experiencing disruption to some of our services. To ensure we can continue to provide care to our patients, some appointments may temporarily be moved to our branch surgery 1 Manchester Road or at the new Fairfield Surgery site (the old Woolston Clinic building WA1 4LS) until our systems are fully restored. Any patients who's appointments are being moved will be contacted by our team.
We sincerely apologise for any inconvenience this may cause and appreciate your patience and understanding during this time.💙
We would also like to extend our thanks to our new neighbours at Fairfield Surgery for accommodating us at such short notice and enabling us to continue seeing patients.

👨🏻‍⚕️We are a Training Practice 👩🏾‍⚕️
Our resident doctors work alongside us for up to 6 months of the year. Resident doctors are fully qualified doctors who have completed medical school- they are simply undertaking speciality training (ST) in a field they wish to pursue (in this case, as a General Practitioner- GP).
By supporting resident doctor training, you are helping us maintain excellent standards of care today while developing the GP’s of tomorrow. 💙

Warrington's Diabetes Support Group is held on a Wednesday each month at the Living Well Hub Warrington, Horsemarket Street, WA1 1XL.
Support sessions are open to any adults living with or affected by different types of Diabetes.
