What is Community SWITCH?

SWITCH (Supporting Wellbeing Improving the Community’s Health) connects you to local support, activities, and services that can help improve your overall health and quality of life. connects charities, the NHS, public sector partners, and community groups to provide joined-up person-centred support.

This complements existing healthcare provision, focusing on a holistic, person-centred "What matters to you?" approach, not merely the traditional "What is the matter with you?"

The Community Switch website is a resource platform signposting to organisations who can support with the below:
• Loneliness
• Feeling stressed or anxious
• Worried about memory loss
• Help with transport
• Carer support
• Neurodivergence
• Bereavement
• Living or recovering from cancer
• Drug, alcohol, or gambling addiction
• Help managing a long term condition
• Support for young people
• Diabetes support
• Menopause or perimenopause

This year we will be offering both flu and covid vaccines to eligible patients. If you have an email or mobile number registered with the surgery, you will receive an invite with a booking link within the next few weeks (if not done so already)! This allows you to book directly into one of our three clinics in October (8th, 15th and 22nd) at The Allendale Centre. We will also attempt calling those who don’t have a either a mobile or email address registered with us.

If you have any queries regarding this, please call our dedicated vaccinations line on 01202 848261 and selecting option 5. Please note, this line is open everyday between 9am-11am and 2pm-4pm.

At the moment, NHS England is undertaking a validation project in regards to GP lists and patient registrations. This is a nationwide scheme and happening at every practice accross the country . Due to this project, you may be sent a letter by either ourselves or from NHS England asking you to confirm your address.

You may get sent a letter if:
- You have not been in contact with the surgery for some time
- You have not responded to screening letters
- You have failed to respond to letters regarding confirmation of your xcontact details.

To ensure you stay registered with the practice, please follow the instructions as stated in the letter.
If there is no response to the letters you are sent, this will result in an automatic removal from the surgery after the stated time period in the letter has passed.

We don't want patients to removed unnecessarily from our care. Please act if you receive a letter.
