Long Term Condition Self Management App

Sheet Street Surgery are excited to be part of the launch of a new Self Management App for Long Term conditions call my mhealth - this includes Asthma, COPD, Diabetes and Cardiovascular Disease.
You may receive an invitation from the practice team offering access to my mhealth if you have a long-term condition that would benefit from access to the app. It's free, and it's completely up to you whether you use it.

My mhealth explains your condition in simple terms and gives you tips and tools to help you feel more confident day to day. It doesn't replace your usual care, your GP practice team are still here for you, just as before. It works alongside them, to help you understand and manage your condition yourself where you can.

If you have any further questions, please discuss with the practice team. Or for further information about the app, the my mhealth Digital Health Advisers will be happy to help.

We are delighted to share the results of the 2026 GP Patient Survey, which highlight the positive experiences of patients using our services and reflect the hard work and dedication of our entire team.

The survey, carried out annually across England, gives patients the opportunity to share their experiences of general practice. This year, Sheet Street Surgery performed exceptionally well, scoring above both the National and Frimley Integrated Care System (ICS) averages in 10 of the 14 key survey measures.

Some of our highest scores included:

95% of patients said it was easy to contact us through our website.
91% rated their overall experience of the practice as good.
94% said they had confidence and trust in the healthcare professional they saw or spoke to.
95% felt their clinician listened to them and had all the information needed to provide their care.
93% felt involved in decisions about their treatment.

These results reflect our commitment to providing accessible, high-quality, patient-centred care. We are particularly pleased that, where we scored above the benchmark, we exceeded the National and ICS averages by an average of around 13 percentage points.

We would like to thank every patient who took the time to complete the survey. Your feedback is invaluable and helps us celebrate what we do well while identifying opportunities to improve further.

This free community event is open to adults who are neurodivergent, supporting a neurodivergent family member, awaiting assessment or diagnosis, recently diagnosed, or seeking to better understand ADHD and autism.

The aim is to provide an informal and supportive environment where attendees can connect with others, access reliable information, and hear from an experienced speaker.

Sheet Street Surgery Walk & Talk

Our first walk and talk took place on Wednesday in a very chilly and foggy stag meadow. Thank you to those who came it was a wonderful way to start the day.

Next walk Wednesday 11th February at 10.30am - we look forward to seeing you there.
