As part of the 80th Anniversary (Oak Anniversary) celebrations of Holy Family Catholic Hospital, Berekum, Management is pleased to invite all past and present staff to contribute to a commemorative publication titled:

*“My Hospital and Me”*

This special book will preserve the memories, experiences, and remarkable contributions of those who have been part of the Holy Family Hospital family over the past 80 years. It is an opportunity to tell your story, celebrate our shared legacy, and inspire future generations.

*What should your write-up include?*

Your submission should have two parts:

*Part One*– My Hospital and Me
Share your personal experience at Holy Family Catholic Hospital, Berekum. Kindly include:

* The years or period you served at the hospital.
* The role or capacity in which you served.
* Your memorable experiences.
* The impact you made on the hospital.
* The impact the hospital has had on your life, career, or personal growth.

Part Two – Author’s Profile
Please include:
