A Whipple procedure — also known as a pancreaticoduodenectomy — is a complex operation to remove the head of the pancreas, the first part of the small intestine (duodenum), the gallbladder and the bile duct.

The Whipple procedure is used to treat tumors and other disorders of the pancreas, intestine and bile duct. It is the most often used surgery to treat pancreatic cancer that's confined to the head of the pancreas. After performing the Whipple procedure, the surgeon reconnects the remaining organs to allow you to digest food normally after surgery.

The Whipple procedure is a difficult and demanding operation and can have serious risks. However, this surgery is often lifesaving, particularly for people with cancer.

Simple liver cysts are fluid-filled cavities in the liver. Usually, they cause no signs or symptoms and need no treatment. However, they may become large enough to cause pain or discomfort in the upper right part of the abdomen.

Most liver cysts can be detected on ultrasound or computerized tomography (CT) scans. When needed, treatment may include drainage or removal of the cyst.

In this particular case, the size of the cyst is causing symptomps to the adjacent organ, so a laparoscopic unroofing of the cyst was performed.

It is very troublesome when biliary tract disease such as bile duct stones are complicated with cholangitis, obstructive jaundice, and pancreatitis. Endoscopic retrograde cholangiopancreatography (ERCP) is the best option in order to remove bile duct stones. This skill for the removal of stones involves the initial common bile duct (CBD) cannulation, subsequent papilla opening broadening. It can be done by either endoscopic sphincterotomy (EST) or endoscopic papillary balloon dilation (EPBD).

In this case we use the EST and EPBD to help extract the huge CBD stone, to relieve the Obstructive jaundice

Extended left Hepatectomy was performed, resecting the segment 4A,4B,2,3 and in this particular case segment 1, due to the Hepatocellular Carcinoma position mainly in the superior of left hemiliver. Glissonean Pedicle Approach still helps to achieve easy hillar control.

The HCC location at segment 1 adheres to the Inferior Vena Cava (IVC) so meticulous separation needs to be performed without injuring the IVC.

Fortunately, oncologic margin was achieved without compromising the liver function, which is proven with NO Post Hepatectomy Liver Failure (PHLF) occurred afterwards. 😁
