ACL Series Part 7

I get asked ‘How is Brendan getting on’ more than I get asked about myself… so here is your long awaited update 🤣🤣

T1 testing results came back, although not breaking any records, he is on track with where we want to be. This rehab is not under any time pressure, we want to take things slow and do them right. Chasing norms you see on social media can be hard and upsetting, it’s worth reminding ourselves there is no race here, this has been a 7 year old injury so rushing to hit strength targets, move onto advanced stage of rehab is not a priority. Coupled with exam stress for Brendan this is not easy.

Everything is where is needs to be for the moment. We will work on a plan going forward to integrate more jumping, hopping and landing mechanics

🇮🇪🏆 Team Pursuit National Champions 🏆🇮🇪

Here is a series of clips from the weekend competing at nationals on the track. When anyone asks about cycling I like to say I am retired but this discipline keeps drawing me back in!

Delighted to defend our title of team pursuit national champs for the 4K pursuit with great team

On a personal note I PBd my time in the 1K Time trial which secured me a bronze medal in the event 🥉

We also competed in team sprint (1.5laps) and the Madison (teams of two race with a sprint every 5 laps, one member of team is resting while the other races, you tag each other in and out with a hand sling 🤝)

Next weekend is Nationals for the individual events which I will be competing in 🤞🏼🍀

PCL is one of the least common knee ligament injuries, the typical mechanism is a fall on hard ground landing directly on the knee (posterior tibial translation) hence its nickname as the ‘dashboard injury’ due to its common mechanism being knee hitting the dashboard in a motor vehicle collision

Jamie’s presentation was similar, a fall on hard ground and an instant shot of knee pain on landing, clinical presentation main findings were a positive posterior drawer test, pain on resisted hamstring flexion deep in knee and a tibial sag. Boney impact injuries ruled out. MRI diagnosis confirmed PCL partial tear and ACL grade 1 sprain.

We broke rehab down into 4 main stages:
Stage 1: We locked in brace to stabilise knee and allow healing of ligaments. This time was spend focusing on restoring ROM, maintaining upper body strength, nutrition and sleep to promote recovery. Reducing pain and swelling. Any ROM work was done with band assist to stop hamstring working, hamstrings pull your tibia backwards - which will pull on the healing PCL

Stage 2 was focused on strength, once swelling, pain and ROM were controlled, we could begin to load up on the weights and build strength, this was monitored with dynamometery to make sure we are addressing specific weak areas. Lots of single leg control and proprioceptive work was done also. Hamstring strengthening began with hip hinging before working knee flexion once tolerated.

Stage 3 was focused on polymerics, to improve his capacity to tolerate pitch work, running and sprinting. We broke down key movement patterns a midfielder needs - broke them down and through repetition worked on them to ensure he could perform each component of playing well before combining them all together on a pitch - when other distractions are around and he cannot ‘think’ about what his knee is doing. Here is where we drilled these movements in place, so they became subconscious and he was happy accepting weight through the knee

Stage 4 was translating all this to the pitch, combining all the previous hard work to sports specific drills. Non contact first before adding controlled contact

Still can’t beat me in a throw in !

How to RDL 🏋️

As part of exercise series with guns I teach the technique of performing an RDL

An exercise which can be tricky to get hang of as it is centred from a hip hinge position - where the hips travel backwards to lower the weight (think car door!!)

As with any exercise, begin light and work on getting the form and tolerance built up in your tissues before going up on the weight! If you have any concerns with this exercise it is no harm to check in with a chartered physiotherapist to see if this is right for you!
