Eczema is not simply “dry skin.”
It’s a biologic barrier disorder driven by chronic inflammation, immune dysregulation, and structural damage to the skin barrier itself.
Healthy skin functions like a sealed biologic wall:
it keeps moisture in and irritants out.
In eczema, that wall becomes disrupted.
Ceramides decrease.
Filaggrin function may become impaired.
Transepidermal water loss (TEWL) increases.
Microscopic fissures form between corneocytes.
Irritants, allergens, and microbes pe*****te more easily.
Then the inflammatory cascade begins.
One of the most important concepts in eczema pathophysiology is the itch–scratch cycle:
itch → scratching → barrier damage → inflammation → more itch
Over time, the skin becomes increasingly reactive, hypersensitive, and chronically inflamed.
Modern research also shows eczema is strongly connected to immune signaling abnormalities, especially Th2-dominant inflammation and cytokine activation.
This is why aggressive skincare often worsens eczema instead of helping it.
The goal is not to “dry the skin out.”
The goal is to:
