On my father’s deathbed, he said one word “Revenge.”
At the time, we were told his death was due to natural causes heart failure, and that was reflected in the original inquest outcome. But we would not accept that.
I spent years investigating my father’s case, reviewing records, obtaining expert opinions, and asking questions.
That journey ultimately led to what I understand was Ireland’s first historic second inquest of its kind, where the original conclusion was overturned and replaced with a verdict of medical misadventure.
The case has since involved engagement with An Garda Síochána, submissions to the DPP, and representations seeking further government review and a non statutory inquiry.
After everything, I decided to write a book.

This is a true story from Limerick about family, loss, determination, and a pursuit of answers.
That journey also led me to establish Medical Records Research Ireland in 2025. Today, I work with families by reviewing medical records, helping identify potential areas of concern, and helping point them toward the appropriate independent experts for further opinion. I currently have 13 active clients.
