This image (-permission granted with focus to educate and increase awareness); is not just a diagnosis — it is a woman. A mother. A sister. A daughter. A friend. A wife. A life.

During pregnancy, she noticed changes that were worse than her first pregnancy.

However, because she had recovered quickly after her first birth, she only mentioned it in passing during antenatal visits. She fully expected to have another natural delivery.

At one of her final antenatal appointments, her Obstetrician assessed her and was alarmed by the severity of the varicose veins and made the decision for a caesarean section.

She attended physiotherapy 15 days postpartum with this presentation and explained that it had reduced significantly and had previously been worse. After two sessions, I drafted a referral letter to an interventional radiologist; supported by these images. It would have also taken a holistic approach if she was referred for review by a vascular specialist. A referral letter had also been drafted by her GP.

She is now 7 months postpartum — and she reports that she was contacted and informed that there was a 2-year waiting list to see a specialist. She is yet to be reviewed.

Thankfully, she remains well, and has made steady improvement.

This case raises critical clinical and systemic questions:

What are the established referral pathways for vulvar varicosities of this severity?

• Who is responsible for follow-up — in my humble opinion all healthcare professionals in contact with the mother.

• What if thrombosis had occurred — would this have been detected on time?

Vulvar varicosities are often dismissed as benign — yet they can thrombose, bleed, and in rare cases become life-threatening.

Women deserve timely assessment, clear referral pathways, and structured postpartum follow-up.

One life is one too many.

, , and save lives.
