كسر معقّد في لوح الكتف لدى شابة في العشرينات

تعرضت شابة في العشرينات من عمرها لإصابة أدت إلى كسر متزحزح في لوح الكتف (Scapula).

نظرًا لطبيعة الكسر ودرجة الإزاحة، تم إجراء عملية جراحية لإعادة الكسر إلى وضعه التشريحي وتثبيته بشكل ثابت باستخدام الصفائح والبراغي.

كسور لوح الكتف التي تحتاج إلى تدخل جراحي تُعد من الإصابات غير الشائعة، وتتطلب تخطيطًا جراحيًا دقيقًا، خاصة لدى المرضى الشباب، بهدف استعادة تشريح الكتف والمحافظة على أفضل وظيفة ممكنة على المدى البعيد.

نتمنى لها الشفاء التام والعودة إلى حياتها ونشاطها الطبيعي قريبًا. 🙏

A complex scapular fracture in a young woman in her 20s

A young woman in her 20s sustained a significantly displaced fracture of the scapula following trauma.

Due to the fracture pattern and degree of displacement, surgical treatment was performed with open reduction and internal fixation, restoring the anatomy and providing stable fixation with plates and screws.

Scapular fractures requiring surgery are relatively uncommon and can be technically demanding. Careful surgical planning and accurate restoration of the anatomy are particularly important in young patients to achieve the best possible shoulder function and long-term outcome.

I’m happy to share our latest publication in the Journal of Shoulder and Elbow Surgery (JSES).

Our study evaluated the reliability of the most commonly used radiographic landmarks for detecting radial head prosthetic overlengthening and found that no single landmark is sufficiently reliable on its own, emphasizing the importance of a comprehensive intraoperative assessment.

تناول هذا البحث موضوع زيادة طول مفصل رأس الكعبرة الصناعي (Radial Head Arthroplasty Overlengthening)، وهو من المواضيع المهمة في جراحة المرفق، لما له من تأثير مباشر على نتائج الجراحة ووظيفة المفصل والمضاعفات المحتملة.

يسعدني أن أشارككم خبر قبول بحثنا العلمي للنشر في مجلة Journal of Shoulder and Elbow Surgery (JSES)، والتي تُعتبر من أبرز وأهم المجلات العلمية المتخصصة في جراحة الكتف والمرفق على مستوى العالم.

تناول هذا البحث إصابات الأنسجة من الجهه الداخلية من المرفق بعد حالات خلع المرفق، وركز على كيفية تقدير درجة الإصابة في (MCL) والأنسجة الداعمة المحيطة به باستخدام صور الأشعة أثناء العملية الجراحية.

أظهرت نتائج الدراسة وجود مؤشرات شعاعية تساعد الجراح على توقع مدى الإصابة في المرابط والاوتار واتخاذ قرارات أكثر دقة فيما يتعلق بإصلاح الأربطة ولاوتار وإعادة الاستقرار للمرفق، مما قد يساهم في تحسين نتائج العلاج للمرضى الذين يعانون من إصابات وعدم استقرار المرفق.

I am pleased to share that our latest research has been accepted for publication in the Journal of Shoulder and Elbow Surgery (JSES), one of the world’s leading journals in shoulder and elbow surgery.

This study investigated medial soft-tissue injuries associated with elbow dislocations and explored how intraoperative radiographic findings can help surgeons anticipate the severity of injury to the medial collateral ligament (MCL) and flexor pronator mass.

Our findings identified radiographic parameters that correlate with the extent of medial soft-tissue damage, providing surgeons with valuable tools to guide intraoperative decision-making regarding ligament and tendon repair or reconstruction. These insights may help improve the management of complex elbow instability and ultimately enhance patient outcomes.

I would like to thank all co-authors and collaborators who contributed to this work. I look forward to continuing our efforts to advance elbow surgery research and improve patient care.

سعيد بالمشاركة في دورة Mayo Elbow Course – Teach the Teachers لهذا العام، والتي ركزت على جراحة تبديل مفصل الكوع (Elbow Arthroplasty). في الولايات المتحدة الأمريكية.

كانت فرصة مميزة للقاء أصدقاء وزملاء من مختلف أنحاء العالم، وتبادل الخبرات والأفكار ومناقشة أحدث التقنيات والتحديات في جراحة الكوع.

مثل هذه اللقاءات العلمية لا تقتصر فقط على التعليم، بل تساهم في تطوير المعرفة وتحسين مستوى الرعاية الطبية التي نقدمها لمرضانا.

Very happy to participate in this year’s Mayo Elbow Course – Teach the Teachers, which focused on Elbow Arthroplasty.

It was a great opportunity to meet friends and colleagues from around the world, exchange knowledge and experiences, and discuss the latest advances and challenges in elbow surgery.

كانت تجربة مميزة ومليئة بالنقاشات العلمية الثرية وتبادل الخبرات مع الزملاء.

أكثر ما لفت انتباهي هو المستوى المميز لأطبائنا المقيمين، سواء في العرض أو المشاركة الفعالة في الجلسات العلمية. نفتخر بجهودهم وتطورهم المستمر، وهم فعلاً مستقبل هذا التخصص.

I’m pleased to have participated in the annual Orthopedic Conference, where we presented 5 of our research projects.
It was a great experience filled with scientific discussions and professional exchange.

What impressed me the most was the outstanding performance of our residents, both in their presentations and their active participation throughout the sessions. We are truly proud of their progress—they represent the future of our field.

هذه الصور قد تلمّح إلى آلية محتملة لتطوّر تشوّه fishtail في المرفق، حيث يبدو أن الجزء الداخلي من الـ trochlea ناقص أو غير متطور. لا يزال غير واضح ما إذا كان السبب هو ردّ غير دقيق للكسر في البداية، أو اضطراب في التروية الدموية لهذه المنطقة، والتي تُعرف بأنها ذات إمداد دموي حساس نسبيًا.

These findings may suggest a possible mechanism for the development of a fishtail deformity, where the lateral aspect of the trochlea appears hypoplastic or absent. It remains unclear whether this is related to initial malreduction or to a disturbance in the vascular supply to this region, which is known to be relatively vulnerable.

In general, pediatric elbow injuries may sometimes be underestimated, partly due to soft tissue elasticity and the cartilaginous nature of the immature elbow, which makes radiographic assessment more challenging. In addition, long-term follow-up is not always consistent. As a result, some patients may present later in life with stiffness or deformity.
