Barbara Variable-Thickness Ring in Keratoconus
I would like to share the case of a 24-year-old woman with bilateral keratoconus who was intolerant to contact lenses and had unsatisfactory vision with glasses , previous CXL in both eyes.
Preoperatively
● Both eyes: UDVA 6/60, CDVA 6/10 with approximately 8 D of astigmatism.
In February 2025, I implanted the Barbara Ring, a variable-thickness intracorneal ring segment that I designed and is manufactured by Mediphacos. Its thickness gradually increases toward the midpoint and then decreases, allowing the thickest part of the segment to be aligned with the steepest area of the cone for a more phenotype-oriented treatment.
At 17 months
● Right eye: UDVA 6/10, CDVA 6/6 with only 2 D of astigmatism.
● Left eye: UDVA 6/15, CDVA 6/6 with only 2.5 D of astigmatism.
The patient is delighted with the outcome and no longer depends on contact lenses or even glasses.

After an excellent congress in Moscow, I had the pleasure of continuing this inspiring scientific journey in Yerevan, Armenia, at a remarkable congress organized by my dear friend Prof. Arrashes Zilfyan.

A special highlight was the keratoconus session organized in collaboration with the International Keratoconus Society, bringing together expertise, passion, and real clinical experience.

The live surgery was outstanding, and the scientific content reflected a very high level of ophthalmology.

It was a true pleasure to meet exceptional colleagues from the Fyodorov Institute in Moscow, Belarus, Lithuania, Georgia, Germany, and my dear friend Miltiadis Balidis, co-founder of the IKS.

Honored to participate in the congress of the Krasnov Institute, where the session on keratoconus was organized jointly by our organization, the International Keratoconus Society, together with the Krasnov Institute.

It was a pleasure to share my experience with CAIRS and to exchange ideas with distinguished colleagues from different countries. The scientific level of the congress was exceptionally high, and the live surgery sessions reflected a truly advanced level of ocular surgery, precision, and innovation.

Meetings like this highlight the importance of international scientific collaboration in advancing the understanding and treatment of keratoconus and corneal disease for the benefit of patients worldwide.

More than 1,000 colleagues from across the world, united by one passion — improving the lives of our keratoconus patients.

Beyond the outstanding science and courses, what truly stood out was the spirit of connection, generosity, and shared purpose.

Facing Keratoconus is Dynamic

In this patient, PMMA intracorneal ring segments (ICRS) implanted in the right eye improved vision and maintained functional sight for many years—remaining better than the left eye, which underwent keratoplasty and additional surgeries, yet still ended with compromised vision.

After 22 years of benefit, one PMMA segment extruded, and when the effect of the remaining segment declined, CAIRS (corneal allogenic intrastromal ring segments) provided a new opportunity to help preserve vision.

Keratoconus management is a dynamic journey, where different technologies over time can complement each other to protect vision and delay more invasive procedures
