When required, EUS can also help guide fluid or tissue sampling for further evaluation.

Early and accurate assessment can help determine the appropriate monitoring or treatment plan.

🩸 𝐏𝐀𝐓𝐈𝐄𝐍𝐂𝐄 𝐈𝐒 𝐓𝐇𝐄 𝐊𝐄𝐘 𝐓𝐎 𝐓𝐀𝐂𝐊𝐋𝐈𝐍𝐆 𝐀 𝐋𝐀𝐑𝐆𝐄 𝐆𝐀𝐒𝐓𝐑𝐈𝐂 𝐕𝐀𝐑𝐈𝐗 𝐖𝐈𝐓𝐇 𝐁𝐋𝐄𝐄𝐃

A 40-year-old male with ethanol-related chronic liver disease and portal hypertension presented with hematemesis and melena.

UGI endoscopy revealed large high-risk esophageal varices with GOV1 large F3 fundal varix bearing an ulcer/stigmata of recent bleeding.

⚠️ 𝐓𝐡𝐞 𝐂𝐡𝐚𝐥𝐥𝐞𝐧𝐠𝐞

A large fundal varix with recent bleeding stigmata can be technically challenging to manage endoscopically bcz risk of bleeding is more due to high portal pressure

EUS-guided coil + glue therapy would have been an alternative approach; however, it could not be performed in this case because of financial constraints.

Therefore, conventional endoscopic cyanoacrylate glue injection was planned.

💉 𝐓𝐡𝐞 𝐊𝐞𝐲 𝐋𝐞𝐚𝐫𝐧𝐢𝐧𝐠 𝐏𝐨𝐢𝐧𝐭 — 𝐏𝐀𝐓𝐈𝐄𝐍𝐂𝐄

During glue therapy of a large fundal varix, controlled and deliberate injection is crucial.

▪️ Glue was injected slowly and carefully rather than as a rapid bolus.
▪️ After the first injection, we waited for adequate polymerization and obliteration of the injected segment.
▪️ The varix was reassessed and a second injection site was identified.
▪️ Glue was then injected at the second site, again allowing adequate time for solidification.

📌 Avoiding an injection directly at the tip of the varix and preferentially targeting the deeper/base portion can help minimize bleeding from the puncture site.

🛑 𝐖𝐡𝐲 𝐒𝐥𝐨𝐰 𝐚𝐧𝐝 𝐂𝐨𝐧𝐭𝐫𝐨𝐥𝐥𝐞𝐝?

Rapid injection of cyanoacrylate may increase the risk of extravasation and systemic embolization. Controlled injection with adequate waiting time allows the glue to polymerize and the varix to progressively solidify.

We had previously encountered significant bleeding during glue injection in a gastric varix, which reinforced an important lesson:

🧠 “𝐃𝐎𝐍’𝐓 𝐑𝐔𝐒𝐇 𝐓𝐇𝐄 𝐆𝐋𝐔𝐄.”

Patience, controlled injection, waiting for adequate solidification, and reassessment between injections are key when dealing with a large fundal varix.

In this case, the fundal varix was successfully solidified/obliterated endoscopically.
