Many women spend years believing the fullness in their underarms is simply “extra fat.” They try dieting, gym workouts, spot reduction exercises, and different clothing styles to hide it—but nothing seems to change.

That’s because in many cases, it isn’t fat at all. It’s accessory (axillary) breast tissue—a developmental variation where breast tissue extends into the underarm. It can become more noticeable after puberty, hormonal changes, pregnancy, or weight fluctuations.

This 24-year-old patient’s biggest concern wasn’t pain—it was confidence. She wanted to wear sleeveless outfits without constantly thinking about the contour of her underarms. The added challenge was asymmetry, with one side being more prominent than the other, so the surgery wasn’t simply about removing tissue—it was about restoring balance.

One aspect patients often overlook is scar planning. In plastic surgery, the operation doesn’t end with removing tissue. Every incision is carefully designed so that, as it heals, it blends into natural skin creases and remains as discreet as possible.

The final result isn’t judged by how much tissue was removed. It’s judged by how natural the contour looks when the arms are by the side, when they’re raised, and how confidently the patient walks out wearing clothes she previously avoided.

At just 28 years old, she was diagnosed with a breast tumour.

While tumor removal was the biggest battle ahead, one question stayed with her: “Will I still look like myself after surgery?”

Her treatment became a collaboration between an oncosurgeon and a plastic surgeon. Every incision was planned together to ensure complete tumour removal while preserving the breast’s appearance as much as possible with all the scars hidden in the natural crease.

After the tumour was removed, she chose fat grafting—using her own fat instead of implants—to restore the breast’s natural shape and contour.

Today, her story is more than a successful surgery. It’s a reminder that breast cancer care isn’t only about removing a tumour—it’s also about helping someone move forward with confidence.

On this World Plastic Surgery Day, I’m grateful to the Indian Journal of Plastic Surgery (IJPS) for giving us the opportunity to share this special case with the world. Every year on 15th July, we celebrate a specialty that is often misunderstood.

Many people think plastic surgery is only about looking better. While aesthetic surgery is an important part of what we do, plastic surgery is so much more than that.

This profession has taught me that every part of the human body matters. A finger is never just a finger. A scar is never just a scar. Every structure has a purpose, and every patient has a story.

Over the years, I’ve had patients share insecurities they’ve never spoken about—not even with their parents, partners, or closest friends. Some have hidden scars for years. Others have struggled with changes after pregnancy, congenital differences, or features that quietly affected their confidence every single day.

By the time they come to us, they’re not simply looking for an operation. They’re looking for understanding, reassurance, and hope.

Plastic surgery is about restoring function, form, and confidence. Whether it’s reconstructing a hand after trauma, treating burns, correcting congenital conditions, performing breast reconstruction, or helping someone feel more confident through aesthetic surgery, every procedure has one common goal—to improve quality of life.

Being a plastic surgeon is a privilege I never take for granted. From head to toe, every operation demands precision, responsibility, and empathy.
