Nitroglycerin (Glyceryl trinitrate)
Nitroglycerin एक potent vasodilator है, जिसका मुख्य उपयोग heart-related chest pain यानी angina में किया जाता है।
Class: Nitrate / anti-anginal
Mechanism: शरीर में nitric oxide (NO) बढ़ाता है → vascular smooth muscle relax करता है → मुख्यतः venodilation → cardiac preload और myocardial oxygen demand कम होती है।
Uses:
Acute angina/chest pain relief
Angina की prevention (formulation के अनुसार)
Hospital setting में कुछ cases में acute heart failure/hypertensive emergencies
Dosage forms: Sublingual tablet/spray, transdermal patch, ointment, IV infusion
Common side effects: Headache, dizziness, flushing, low BP, कभी-कभी reflex tachycardia.
Important interaction: Sildenafil, tadalafil जैसे PDE-5 inhibitors के साथ nitroglycerin नहीं लेना चाहिए, क्योंकि BP dangerously low हो सकता है.

Ivabradine एक दवा है जो मुख्य रूप से heart rate कम करने के लिए उपयोग की जाती है।
Ivabradine – मुख्य जानकारी
Drug class: If-channel (funny current) inhibitor
मुख्य उपयोग: Chronic stable angina और कुछ patients में chronic heart failure
Mechanism: SA node में If current को inhibit करके heart rate कम करती है।
खास बात: यह heart rate कम करती है लेकिन सामान्य therapeutic doses पर contractility और blood pressure पर comparatively कम direct effect डालती है।
Common side effects:
Bradycardia (heart rate बहुत कम होना)
Headache
Dizziness
Phosphenes — आंखों के सामने चमक/brightness जैसी visual sensation
महत्वपूर्ण: बहुत कम heart rate, कुछ conduction disorders और कुछ cardiac conditions में इसका उपयोग contraindicated हो सकता है।

Drug Class: Cardiac glycoside
Main uses:
❤️ Heart failure – symptoms/control में मदद
💓 Atrial fibrillation (AF) – heart rate control करने में मदद
Mechanism:
Digoxin Na⁺/K⁺-ATPase को inhibit करता है → intracellular Ca²⁺ बढ़ता है → force of heart contraction बढ़ती है (positive inotropic effect). साथ ही vagal activity बढ़ाकर heart rate/AV conduction कम करता है।
Common side effects:
Nausea, vomiting, diarrhea
Dizziness, weakness
Bradycardia/arrhythmias
Visual disturbances (जैसे blurred/yellow vision)
⚠️ Important: Digoxin का therapeutic index narrow है, इसलिए toxicity का risk रहता है। Kidney function और कुछ electrolytes (खासकर potassium) महत्वपूर्ण होते हैं। Dose हमेशा डॉक्टर की सलाह से लें।

Hydrochlorothiazide (HCTZ) एक Thiazide diuretic (थायाज़ाइड डाययूरेटिक) है।
मुख्य उपयोग:
🩺 High BP (Hypertension) को नियंत्रित करने में
💧 Edema (शरीर में अतिरिक्त पानी/सूजन) कम करने में
❤️ Heart failure में fluid retention कम करने के लिए कुछ परिस्थितियों में
Mechanism:
यह kidney के Distal Convoluted Tubule (DCT) में Na⁺/Cl⁻ cotransporter को inhibit करता है → sodium और पानी का excretion बढ़ता है → blood volume और BP कम होते हैं।
Common side effects:
बार-बार पेशाब आना
चक्कर/कम BP
Hypokalemia (पोटैशियम कम होना)
Sodium कम होना
Uric acid बढ़ना
Blood glucose बढ़ सकता है
⚠️ Drug का उपयोग डॉक्टर की सलाह के अनुसार ही करें।

Furosemide एक Loop Diuretic (लूप डाययूरेटिक) दवा है, जो शरीर से अतिरिक्त पानी और सोडियम बाहर निकालने में मदद करती है।
💊 Furosemide – मुख्य जानकारी
Drug class: Loop diuretic
मुख्य mechanism: Kidney के Na⁺-K⁺-2Cl⁻ cotransporter (NKCC2) को inhibit करता है।
मुख्य उपयोग:
❤️ Heart failure में edema
🫁 Pulmonary edema
🩺 Hypertension में कुछ परिस्थितियों में
🦵 Liver/kidney disease से होने वाली fluid retention
Effect: पेशाब की मात्रा बढ़ाता है और शरीर का अतिरिक्त fluid कम करता है।
⚠️ Common side effects
बार-बार पेशाब आना
चक्कर/low BP
Potassium कम होना (hypokalemia)
Sodium कम होना
Dehydration
कमजोरी या muscle cramps
Important: Furosemide से electrolyte imbalance हो सकता है, इसलिए इसे डॉक्टर की सलाह और जरूरत पड़ने पर electrolyte/kidney-function monitoring के साथ लेना चाहिए।
