*THURSDAY THYROIDS* 🦋

The neck said: “Not much.”
The operating room said: “Think again.”

Today’s thyroid surgery turned out to be quite the surprise.

What looked relatively modest externally was actually a massive multinodular thyroid with retrosternal extension.

And this is where thyroid surgery becomes less about size… and more about anatomy, experience and meticulous dissection.

Both RLNs preserved.
All Four Parathyroids preserved.
No gross intraoperative features of malignancy.

And then came the total thyroidectomy specimen 🦋🦋🦋

Blessed to be taught by the best. 🙏

THURSDAY THYROIDS 🦋 | When Size Meets Precision

Not every thyroid surgery is routine.

This patient presented with a massive multinodular goitre, occupying almost the entire front of the neck. Despite its size, the goal was never just to remove the gland- it was to do so safely.

✅ Total Thyroidectomy performed

Throughout the surgery, meticulous dissection was carried out to preserve the structures that matter most:
🔹 Both recurrent laryngeal nerves (voice nerves)
🔹 All parathyroid glands (which regulate calcium levels)

Every journey to a conference begins with a lecture schedule, but the memories are made through the people you meet along the way.

It was an absolute pleasure to be part of ThuyilCon 2.0 in KH Apollo Hospital (Ranipet), Tamil Nadu, as a Guest Speaker.

I had the opportunity to speak on:
🔹 Sleep Lab Revolution & Sleep Studies (Polysomnography)
🔹 Paediatric Obstructive Sleep Apnea

Also, conducted a Hands-on Coblation Workshop, sharing techniques, exchanging ideas, and teaching enthusiastic colleagues.

What made this meeting truly special was reconnecting with my mentors, meeting old friends, and making new ones within the ever-growing ENT and Sleep Medicine family. These interactions remind us that medicine is built not only on science, but also on mentorship, collaboration, and lifelong friendships.

Heartfelt thanks to my dear friend Dr. Sankara, Organising Secretary of ThuyilCon 2.0, for the gracious invitation and for curating such an exceptional scientific meeting. Your passion and hospitality were evident in every detail.

Feb'26: Seeing INDIA 🇮🇳 alongside my name in an international faculty panel was a moment of immense pride, gratitude and responsibility.

Glad to be a part of ISSS- IASSACON 2026, the International Sleep Surgery Conference, as Invited Faculty. Had the opportunity to deliver 2 talks, and conduct an Instructional Course on Sleep Endoscopy.

The icing on the cake was being awarded 3 medals at an international platform - 1 Gold 🏅 and 2 Silver 🥈🥈- a truly memorable milestone.

But beyond the academics and recognitions, the most special part of the conference was reconnecting and collaborating with the incredible sleep surgery fraternity from India and across the globe 🌎

Experiences like these inspire us to continue pushing boundaries, advancing sleep care, and proudly representing Indian ENT on the global stage. 🇮🇳✨

A coin. 🪙
A curious child. 👶
A silent emergency. 🚨

This 2-year-old presented with a coin stuck in the food pipe (esophagus) — a situation that can quickly turn serious if missed.

🔍 X-ray confirmed the diagnosis
🔬 Intra-op view showed impaction in the upper esophagus
✔️ Successfully removed endoscopically

💡 Key message:
Not every swallowed object reaches the stomach safely.
The esophagus is the most common site where coins get stuck in children.

⚠️ Watch for warning signs:
• Sudden drooling
• Difficulty swallowing
• Vomiting
• Refusal to eat

🚨 If suspected, do not wait. Seek urgent medical care.

Early intervention = safer removal + fewer complications.

We spend almost one-third of our lives sleeping, yet it is the most neglected pillar of health.

As a sleep doctor, I often meet patients who have normalized years of poor sleep.
They think constant fatigue is normal.
They think loud snoring is harmless.
They think waking up exhausted is just part of a busy life.
The think diabetes & hypertension are a constant accomplice.

But very often, these are warning signs.

One of the most commonly overlooked conditions is Obstructive Sleep Apnea (O.S.A.) where breathing repeatedly stops during sleep. What many people dismiss as 'just snoring' can silently affect the heart, brain, blood pressure, metabolism, and overall quality of life.

The good news is that once identified, sleep disorders are highly treatable, and the improvement patients experience can be life changing: better energy, sharper focus, improved cardiovascular health, and truly restorative sleep.

This World Sleep Day, take a moment to ask yourself a simple question:
Are you sleeping… or just lying in bed through the night?
