Acute Poisoning: Initial Emergency Management

In poisoning emergencies, stabilization comes first. Use the ABCDE approach, identify the toxin and route of exposure, prevent further absorption, administer a specific antidote when indicated, and monitor continuously for deterioration.

Key nursing priorities:
• ABCDE assessment
• Poison identification
• Safe decontamination
• Activated charcoal considerations
• Specific antidotes
• Continuous monitoring
• Patient and staff safety

A structured approach can help recognize life-threatening complications early and support timely emergency care.

Beta-Blockers Made Simple

Beta-blockers reduce sympathetic stimulation, helping to slow the heart rate, lower blood pressure, and reduce cardiac workload.

Key nursing points:
• Monitor HR and BP
• Watch for bradycardia and hypotension
• Use caution with bronchospastic disease
• Never stop long-term therapy abruptly
• Monitor for masked hypoglycemia in diabetic patients

A high-yield pharmacology topic every nursing student should know.

Acute Respiratory Distress Syndrome (ARDS) — a critical-care emergency every nurse should recognize early.

Key nursing priorities:
• Recognize refractory hypoxemia
• Use lung-protective ventilation
• Monitor PEEP and plateau pressure
• Understand the role of prone positioning
• Maintain careful fluid balance
• Monitor ABGs, SpO₂ and ventilator parameters
• Prevent complications such as VAP, barotrauma and pressure injuries

Save this for your Critical Care Nursing revision.

🚨 HELLP SYNDROME — THE OBSTETRIC EMERGENCY YOU CANNOT MISS! 🤰🩸

A pregnant patient presents with right upper-quadrant pain, nausea, thrombocytopenia and elevated liver enzymes.

What is the diagnosis?

👉 HELLP SYNDROME

🇺🇸 USMLE | 🇬🇧 PLAB/MRCP | 🇮🇳 NEET-PG/INI-CET | 🌍 MBBS

HELLP is a serious pregnancy complication characterized by:

🔴 H — Hemolysis
🟠 EL — Elevated Liver enzymes
🟣 LP — Low Platelets

---

🧠 WHY DOES HELLP HAPPEN?

Abnormal placentation and placental ischemia can contribute to systemic endothelial dysfunction.

⬇️

Endothelial injury + platelet activation

⬇️

Microvascular thrombosis

⬇️

🩸 Microangiopathic hemolysis
🧬 Hepatic injury
🩸 Thrombocytopenia

---

🔬 H — HEMOLYSIS

Typical laboratory clues:

• ↑ LDH
• ↑ Indirect bilirubin
• ↓ Haptoglobin
• ↓ Hemoglobin
• Schistocytes on peripheral smear
• Reticulocytosis may occur

The hemolysis is generally microangiopathic.

---

🫀 EL — ELEVATED LIVER ENZYMES

Hepatic injury may produce:

• ↑ AST
• ↑ ALT
• ↑ LDH

🚨 A classic clinical clue is:

Right upper-quadrant or epigastric pain

Severe hepatic involvement can rarely lead to:

⚠️ Subcapsular hepatic hematoma
⚠️ Hepatic rupture
⚠️ Hepatic infarction

---

🩸 LP — LOW PLATELETS

Platelets are consumed within the damaged microvasculature.

A commonly used diagnostic threshold is:

Platelets

A nurse is caring for a 68-year-old patient with a history of left-sided heart failure. The patient suddenly develops severe dyspnea, bilateral crackles, and pink, frothy sputum. The respiratory rate is 34/min, and oxygen saturation is 82% on room air. The patient is anxious and unable to lie flat.

Question:

Which nursing intervention should the nurse perform first?
