The little dreams I once had for myself feel so different when I look back at them now 💖

Somewhere along the way, I became the woman and the doctor I once hoped I would be 🧿

And today, one of the greatest privileges of being a fertility doctor is getting to be part of someone else’s most deeply held dream helping them become a parent. 🤍

Medicine gave me a profession.
Fertility gave that profession a much deeper meaning.

Recently, one of my patients told me, “My embryo transfer didn’t result in pregnancy. My IVF failed.”

But when I reviewed her reports and looked at the entire cycle, I realised this wasn’t simply a failed IVF cycle.

Here’s what we actually found:

1. The embryo had been transferred, but implantation had not occurred.
A negative pregnancy test only tells us that implantation did not result in a pregnancy; it does not, by itself, explain why the embryo failed to implant.

2. Her uterine lining needed to be looked at more closely.
We reviewed the thickness, pattern and preparation of her endometrium to understand whether the uterus was adequately prepared to receive the embryo.

3. The progesterone timing needed to be assessed as part of the transfer preparation.
Since progesterone helps prepare the endometrium for implantation, its timing in relation to the embryo transfer is an important part of evaluating the cycle.

4. We couldn’t assess the outcome by looking at the embryo alone.
Her hormonal, metabolic and uterine factors also needed to be considered, because implantation depends on the interaction between the embryo and the uterine environment.

5. The next step was not simply to repeat the same IVF cycle.
Instead, we needed to understand what happened during this transfer, identify any factors that could be addressed, and then decide whether the next cycle needed a different approach.

This is why I always tell my patients one unsuccessful embryo transfer is an outcome, not a complete explanation.

Before calling it a “failed IVF,” we need to understand what happened during that particular cycle and what can be learned from it.

The pattern I repeatedly see in women with low AMH isn’t simply “fewer eggs”.

It’s waiting too long because of what the AMH report seems to say.

After 15+ years as a fertility specialist and helping 1,000+ women through their fertility journeys, I’ve seen many women panic the moment they hear, “Your AMH is low.”

But low AMH primarily tells me about ovarian reserve not whether pregnancy is impossible.

📌 What matters next is the bigger clinical picture:

* Your age
* Antral follicle count (AFC)
* How your ovaries respond to stimulation
* The number and maturity of eggs retrieved
* Embryo development and quality
* S***m factors
* Your overall reproductive history

If you’ve been told you have low AMH, don’t let one number define your fertility journey.

AMH 0.1 vs AMH 1.0 what does the difference actually mean?
As a fertility doctor, this is something patients ask me very often after seeing their AMH reports.

Many women assume that if their AMH is 0.1 while someone else’s is 1.0, their chances of pregnancy must be dramatically lower.

But fertility doesn’t work that way.

When I evaluate a patient with low AMH, I look beyond that one number. Two things are particularly important:

1️⃣ Ovarian reserve is not the same as pregnancy chances.
AMH gives us information about ovarian reserve and can help us anticipate how the ovaries may respond during IVF. But it does not directly measure egg quality or predict pregnancy on its own.

We look at her age, AFC, ovarian response, previous treatment history, and overall fertility profile to understand what the AMH actually means for her.

2️⃣ Egg quality matters more than the AMH number alone.
Especially in IVF, our goal is not simply to collect as many eggs as possible. We want to maximize the chances of developing healthy embryos, where egg quality, s***m quality and embryo development play a crucial role.

So, an AMH of 0.1 may indicate a lower ovarian reserve than 1.0 but it does not mean your chances of pregnancy are ten times lower.

One AMH value should never define your fertility journey. The complete picture matters.

This couple came to my clinic after 3 failed IVF cycles.

She was 35. He was 37.

By the time they met me, they weren’t just carrying medical reports they were carrying years of disappointment, unanswered questions, and emotional exhaustion.

They had already decided that this would be their last IVF attempt.

Instead of immediately repeating the same protocol, we took a step back and asked a simple question:

“Why did the previous IVF cycles fail?”

After a detailed evaluation, we found a few important areas that needed attention:
✔️ Optimizing the uterine environment before embryo transfer.
✔️ Identifying and treating factors that could affect implantation.
✔️ Personalizing the IVF protocol instead of repeating the same approach.

Only after addressing these factors did we move ahead with the next cycle.

And this time…

✨ They conceived.

Today, they are proud parents to a beautiful, healthy baby girl. 💕

Every IVF failure is heartbreaking, but it doesn’t always mean the journey is over. Sometimes, it means it’s time to stop repeating the same approach and start looking for the missing pieces.

For years, she believed that her low AMH was the only reason she wasn’t getting pregnant.

She had a sedentary lifestyle, was under constant stress, and every consultation revolved around just one blood test AMH.

When she came to me, I took a step back and evaluated her fertility as a whole!

The two most important things that had been overlooked were:

1️⃣ Individualized fertility planning - not just the AMH report.
AMH tells us about your ovarian reserve, but it doesn’t decide your chances of pregnancy on its own. We correlated her AMH with her age, Antral Follicle Count (AFC), ovarian response, and previous treatment history to choose the right treatment strategy instead of assuming poor outcomes based on one report.

2️⃣ Embryo potential over egg count. - Many couples focus only on the number of eggs. As fertility specialists, our goal is to maximize the chances of obtaining healthy embryos, because egg quality, s***m quality, and embryo development ultimately have a much greater impact on pregnancy than the AMH value alone.

One report should never define your fertility journey. The right diagnosis comes from looking at the complete picture not just a single number.
