During consultations, many of my patients with PCOS ask me:
“Doctor, what should I do to get pregnant?”

Here’s what I tell them

1) Don’t focus only on the PCOS diagnosis
Many women assume PCOS is the only reason they’re not conceiving. But fertility is influenced by multiple factors, including age, egg quality, fallopian tube health, and s***m parameters. A complete fertility evaluation is always important.

2) Irregular periods usually mean irregular ovulation
If you’re not ovulating regularly, your chances of conceiving each month decrease significantly. Identifying whether and when ovulation is occurring is one of the first steps in planning the right fertility treatment.

3)Time matters more than most people realize
If you’re under 35 and have been trying for a year or over 35 and trying for 6 months don’t keep waiting. Early intervention often means simpler treatments and better pregnancy outcomes.

4) PCOS is treatable, but treatment should be individualized
Some women may conceive with ovulation induction alone, while others may require IUI or IVF. The best treatment depends on your age, ovarian reserve, duration of infertility, and overall fertility profile.

The key is getting the right diagnosis and treatment plan at the right time. ✨

Even when basic semen parameters appear normal, advanced s***m factors may influence IVF outcomes.

Many women who walk into my clinic tell me, “Doctor, my egg count is good and my AMH levels are normal... then why am I still not getting pregnant?”

The truth is, fertility is about much more than just these two numbers.

Here are a few reasons pregnancy may still not be happening:

1. Egg quality matters too
A good egg count doesn’t always mean the eggs are genetically healthy. Age and other factors can affect egg quality and embryo development.

2. The s***m factor is often overlooked
Even when reports appear “normal,” issues related to s***m DNA quality can impact fertilisation and implantation.

3. The fallopian tubes may not be functioning properly
Blocked or damaged tubes can prevent the egg and s***m from meeting naturally.

4. The uterus needs to be receptive
Polyps, fibroids, adhesions, or implantation-related issues within the uterus can make it difficult for an embryo to attach.

5. Hidden hormonal or immune factors
Conditions like thyroid disorders, insulin resistance, endometriosis, or other underlying issues can quietly affect your chances of conception.

If you’ve been trying for a while despite “normal” reports, please know that you deserve a deeper evaluation not self-blame.
