The Fertility Doctor Who Stopped Selling IVF ❤️

A few years ago, I learned a powerful lesson:

Patients don’t come looking for IVF.

They come looking for answers.

They come looking for hope.

They come looking for someone they can trust.

So I stopped focusing on “selling treatment” and started focusing on educating, listening, and guiding.

Sometimes the right answer was IVF.

Sometimes it was natural conception.

Sometimes it was simply giving a couple clarity about their next step.

Something remarkable happened.

Not every patient chose treatment.

A Day in the Life of a Fertility Doctor

Most doctors fight disease.
A fertility doctor fights uncertainty.

Every morning begins with hope.

A woman with low AMH asks,
“Doctor, do I still have a chance?”

A husband quietly wonders,
“Am I the reason we cannot become parents?”

A couple walks in carrying reports, fears, expectations, and sometimes years of heartbreak.

And every day, a fertility doctor stands between science and emotion.

We celebrate pregnancies.
But what people rarely see are the nights after failures.
The negative beta-hCG.
The embryo that stopped growing.
The perfect embryo transfer that still did not implant.
The miscarriage after hearing the heartbeat.
The tears in the consultation room.
The silence when words are not enough.

Unlike many specialties, fertility medicine offers no guarantees.

You can perform the perfect stimulation.
You can retrieve beautiful eggs.

Time is not on pause.
And neither is your fertility.

You can delay decisions.
You can delay plans.
But you cannot delay biology.

AMH is not just a number.
It is your ovarian reserve… your reproductive timeline.

And the hardest truth?

👉 It declines silently
👉 It doesn’t give warnings
👉 It doesn’t wait for the “right time”

Most women I meet are:
✔ successful
✔ settled
✔ “planning later”

But when they finally check…

…it’s often already lower than expected.

And then the question comes:

“Why didn’t I know this earlier?”

Let me tell you this clearly:

Protecting AMH is not about rushing pregnancy.
It is about protecting your options.

Because when you have options:

* you have control
* you have confidence
* you have peace of mind

And when you don’t…
even time feels expensive.

So if you are in your late 20s or 30s:

✔ Check your AMH
✔ Understand your timeline
✔ Plan smart (not late)

🔬 Low AMH: A Lab Report, Not a Life Sentence

I see this every week in clinic.
A woman walks in with her AMH report.
The number is low.
Her face says just one thing: “Am I finished?”

Let’s clear this up with science and sanity.

A 2021 paper in Obstet Gynecol Sci looked at women labelled with diminished ovarian reserve (high FSH, low AMH or low AFC) and checked their eggs after stimulation.

They found:
• Low AMH → fewer mature oocytes
• High FSH → more immature oocytes
• Low AFC → more abnormally shaped oocytes

That’s it. The real message was:
👉 “Low reserve markers = fewer / sometimes less mature eggs in an IVF cycle”
Not:
❌ “You are infertile”
❌ “You must do IVF immediately”



So what is AMH actually saying?

AMH is a stock meter, not a success meter.

It tells us:
• How many follicles are likely to grow if we stimulate
• How strongly your ovaries may respond to injections

It does not tell us:
• Whether your tubes are open
• Whether s***m is normal
• Whether your uterus is receptive
• Exactly who will or won’t get pregnant naturally

And remember:
🕰 Age drives egg quality (aneuploidy).
📉 AMH reflects egg quantity (how many chances per cycle).

A 28-year-old with AMH 0.4 and a 43-year-old with AMH 0.4 do not have the same prognosis. Same number, different biology.



The real myth: “Low AMH = IVF now”

“Low AMH” on its own is not an IVF mandate.

What it really means is:
• We shouldn’t ignore time
• We may get fewer eggs per cycle
• We must be more intentional with the plan

Sometimes that plan is: optimize natural attempts + mild support.
Sometimes it’s IUI.
Sometimes, given age + duration + male factor, IVF earlier is the wiser move.

But the decision should come from the full story, not one hormone.



Here’s how I explain it to my patients:

“Low AMH means your egg supply is on the lower side for your age, so you may produce fewer eggs in a treatment cycle. It does not mean you can’t get pregnant, and it does not automatically mean you need IVF. It means we should be more intentional about timing, protocol, and overall fertility strategy.”

And this is the line I wish every woman could read when she sees her report:

“Low AMH is a timing signal, not a life sentence. It tells us to act mindfully – considering age, partner’s s***m, tubes, health, work–family rhythm – so we don’t lose the window.”

Let’s replace fear of a number with clarity, planning and hope. 🌱
