🧠 Neurofibromatosis — Short Explanation

Neurofibromatosis (NF) is a genetic disorder that affects the nervous system and skin, causing tumors along nerves.

1️⃣ NF1:

Most common type

Café-au-lait spots

Neurofibromas

Axillary/inguinal freckling

May have optic pathway involvement

2️⃣ NF2-related schwannomatosis:

Mainly affects nerves involved in hearing and balance

Vestibular schwannomas

Hearing loss, tinnitus, balance problems

NF1 → Skin + Neurofibromas
NF2 → Hearing + Balance

🌡️ Fever — Pathophysiology

Infection/inflammation 🦠
↓
Immune cells release pyrogens → mainly IL-1, IL-6, TNF-α
↓
These stimulate PGE₂ (Prostaglandin E₂) production in the hypothalamus
↓
Hypothalamic set-point ↑ 🧠
↓
Body perceives current temperature as too low
↓
Vasoconstriction + shivering 🥶
↓
Heat production ↑ → Body temperature ↑ 🌡️
↓
FEVER
When PGE₂/set-point returns toward normal → vasodilation + sweating 💦 → temperature falls.
One-line memory:
Pyrogens → PGE₂ → Hypothalamus set-point ↑ → Shivering → Fever.

⚡ VASOPRESSORS — EASY REVISION! 🏥💉

Think: “Vaso = Vessel, Press = Pressure”
Most vasopressors help support blood pressure by increasing vascular tone and/or cardiac function.

1️⃣ NOREPINEPHRINE — “NO.1 in Septic Shock” 🦠
➡️ Common first-line vasopressor for septic shock
➡️ Mainly increases vascular tone → ↑ BP

2️⃣ EPINEPHRINE — “Emergency Power” ❤️‍🔥
➡️ Cardiac arrest
➡️ Severe anaphylaxis
➡️ Strong effects on heart + blood vessels

3️⃣ PHENYLEPHRINE — “Pure Pressure” 🩸
➡️ Predominantly α₁ agonist
➡️ Causes vasoconstriction → ↑ BP
➡️ Useful in selected hypotensive states

4️⃣ DOBUTAMINE — “Boosts the Beat” ❤️
➡️ Primarily an inotrope
➡️ Increases myocardial contractility
➡️ Used when cardiac output needs support, such as selected cases of cardiogenic shock

5️⃣ DOPAMINE — “Dose-Dependent Action” 🔄
➡️ Effects vary with dose
➡️ Can increase heart rate, contractility and vascular tone
➡️ Its routine use depends on the clinical situation

6️⃣ VASOPRESSIN — “Vessel Tightener” 🚨
➡️ Causes vasoconstriction through V1 receptors
➡️ May be added in vasodilatory/septic shock
➡️ Also has an important role in diabetes insipidus

🧠 Quick Memory:
NORE = Septic shock
EPI = Arrest + Anaphylaxis
PHENY = Pressure
DOBU = Contractility
DOPA = Dose-dependent
VASO = Vasoconstriction

📌 Remember: Drug choice, dose and infusion rate depend on the patient's condition and hospital protocol. Always follow the treating physician's order and local guidelines.

EJV Cannulation

EJV = External Jugular Vein

Use: Difficult peripheral IV access.

Position: Supine, head turned opposite side.

Site: EJV over sternocleidomastoid muscle.

Technique: Aseptic → bevel up → shallow angle → blood return → advance catheter → flush & secure.

Complications: Hematoma, infiltration, infection, thrombophlebitis, arterial puncture.
