Why is consulting the correct speciality very important?
Presenting 3 serious examples , seen in a single day..
Case 1..
A 23 year old male, developed tingling numbness in legs with walking difficulty of sudden onset without any pain..was admitted at a multi-speciality hospital, mri spine done and diagnosed and treated for slipped disc!! From history itself, this was nowhere near a slipped disc. Further probing showed that he had once vomited, and I found weakness of the upper limbs, deglutition problem, speech fatigue, sleepy eyes..etc.. it was a dangerous case of GBS, viral affection of nerves, and fast progressing to a serious stage.. I immediately got him admitted under the care of a neurologist...I hope he recovers as it is still in the treatable stage.
Case 2
60 year old male, walking difficulty with imbalance. Again, labelled lumbar spine disease and advised surgery by non neuro spine doctors. Actually he had parkinsonism and mri spine was not matching complaints. Again, referred him to a neurologist with advice to continue life long care for good control and minimise disability.
Case 3
22 year old young girl, my old patient, consulted me for headaches which appeared pure psychogenic. Father said, I had treated her for over a month of ICU and aggressive care for critical serious head injury in her early childhood (I didn't recollect). But, her physical appearance and thick voice raised a doubt of hormonal disturbances. And she confirmed that she is already on a thyroid replacement. At this age, this raises a doubt of a pituitary gland tumor. I got her MRI done which was reported normal but I found an enlarged gland area. Discussed with the radiologist and got a contrast done to find a micro tumor. Still not needing surgery, I referred her to an endocrinologist for further evaluation and follow up with me as well.
