Depression Is Not Always “Just in the Mind”

Science is increasingly showing that, in some people, depression may involve changes in the immune system and persistent low-grade inflammation.

When inflammatory signals increase, the brain’s immune cells—including microglia—may become activated. Cytokines and related immune signals can then influence the brain circuits and chemical systems involved in mood, motivation, energy, sleep and concentration.

This may partly explain why depression can sometimes feel like having the flu without the fever:

You feel exhausted.
Your mind becomes cloudy.
Nothing feels rewarding.
You become irritable.
Even simple tasks feel overwhelming.

This experience is often called “brain fog.” It is real—but it is complex. Inflammation may contribute to it, alongside disturbed sleep, stress hormones, altered brain-network activity, medication effects and several medical conditions.

The vagus nerve forms an important communication highway between the brain, heart, lungs, gut and immune system. Through what is known as the cholinergic anti-inflammatory pathway, vagal activity may help regulate excessive inflammatory signalling.

Slow, controlled breathing can support parasympathetic regulation and improve autonomic balance. Vagus nerve stimulation goes a step further by directly influencing vagal pathways and interconnected mood-regulating networks.

Implanted vagus nerve stimulation has already been approved in the United States for selected patients with chronic or recurrent treatment-resistant depression. Non-invasive approaches, including transcutaneous vagus nerve stimulation, are opening promising new areas of research—but they must be used responsibly, with proper clinical assessment and realistic expectations.

Sleep is equally important. During restorative sleep, the brain’s glymphatic system participates in the movement and removal of metabolic waste. Good sleep therefore supports brain maintenance, emotional regulation and cognitive clarity—although scientists are still refining exactly how this clearance system works in humans.

From Epilepsy to Depression… From the Brain to the Gut… The story of Vagus Nerve Stimulation is still unfolding.

The journey of Vagus Nerve Stimulation (VNS) began more than three decades ago. Initially developed as an implantable device for drug-resistant epilepsy, it received its first regulatory approval in the 1990s after demonstrating meaningful reductions in seizure frequency.

Researchers soon noticed something unexpected—many patients experienced significant improvements in mood, independent of seizure control. This observation led to further research, culminating in approval for treatment-resistant depression (TRD) in the early 2000s.

Today, the science has expanded beyond implanted devices. Transcutaneous Auricular Vagus Nerve Stimulation (taVNS) offers a non-invasive method of stimulating the auricular branch of the vagus nerve through the external ear, making neuromodulation more accessible and patient-friendly.

What makes the vagus nerve remarkable is that nearly 80% of its fibers carry information from the body to the brain, forming a crucial communication network between the brain, heart, lungs, immune system, and gastrointestinal tract. By influencing these pathways, VNS has opened new possibilities far beyond traditional psychiatry.

When Keeping “Everything” Isn’t Just a Habit…

Many people think hoarding simply means being untidy or emotionally attached to old belongings. In reality, hoarding can sometimes be a clinical sign of a serious psychiatric illness.

In individuals with psychosis, the brain may assign unusual importance to ordinary or discarded objects. Small stones, wrappers, pieces of paper, coins, plastic packets, or other seemingly worthless items may be carefully collected, hidden, and protected. To others, these objects appear to be rubbish. To the affected person, they may hold special meaning, provide a sense of safety, or be linked to beliefs that cannot be explained by logic.

When hoarding occurs alongside symptoms such as:
• Suspiciousness or feeling others are against them
• Talking or laughing to themselves
• Hearing voices
• Social withdrawal
• Neglect of personal hygiene
• Disturbed sleep
• Unusual or bizarre behaviour

…it should never be dismissed as a mere “bad habit” or “stubbornness.” It may indicate an underlying psychotic disorder requiring professional assessment.

It is also important to understand that not all hoarding is psychosis. Hoarding can occur in conditions such as hoarding disorder, obsessive-compulsive disorder, dementia, autism spectrum disorder, and certain neurological illnesses. The surrounding symptoms and clinical context help clinicians arrive at the correct diagnosis.

Early recognition is crucial. Timely treatment often leads to significant improvement, not only in hoarding behaviour but also in the person’s overall functioning and quality of life.

A simple dietary change could completely reverse the dementia-like decline. .

A medical case report published in BMJ Case Reports outlines the dramatic recovery of a 61-year-old woman who lived with a misdiagnosis of dementia for five years. The patient had exhibited severe behavioral changes, psychotic hallucinations, and seizures. Her family observed her talking to spirits and walking naked in public. Initial neurological testing and CT scans showed no typical abnormalities, leading doctors to treat her with standard anti-seizure medication while her cognitive decline worsened. However, when she was referred to psychiatry specialists in Lisbon, a routine blood test revealed the root of her severe neurological distress was not an irreversible brain disease, but an extreme vitamin B12 deficiency.

The deficiency had triggered pernicious anemia, hindering her body's ability to produce healthy red blood cells and damaging the protective myelin sheath around her spinal cord. Following targeted B12 injections and antipsychotic therapy, the patient made a stunning and complete recovery. Her speech became coherent, her psychosis disappeared, and she regained full independence in her daily life. While cognitive decline paired with seizures is an incredibly rare presentation of vitamin B12 deficiency, doctors urge clinicians to rule out vitamin levels in patients presenting with unexplained cognitive deterioration, noting that even prolonged neurological damage can sometimes be entirely reversed.

A Seizure-Control Drug Can Sometimes Affect the Gums

This striking image shows phenytoin-induced gingival hyperplasia, a known but often overlooked complication of long-term phenytoin treatment for epilepsy.

Phenytoin can stimulate gingival fibroblasts to produce excessive connective tissue while reducing normal collagen breakdown. When combined with poor oral hygiene and chronic inflammation, the gums may gradually enlarge and even cover the teeth.

The good news is that this condition is often preventable and treatable through:
