Subchondral Bone Lesion – Understanding the Knee

A subchondral bone lesion is an abnormal area affecting the bone located directly beneath the articular cartilage of the knee joint. It may develop due to repetitive stress, trauma, cartilage damage, osteoarthritis, or other underlying bone and joint conditions.

Depending on its cause and severity, a subchondral lesion may lead to localized knee pain, swelling, tenderness, stiffness, and discomfort during weight-bearing activities. In some cases, the underlying bone may show changes such as bone marrow edema, sclerosis, cystic changes, or structural damage.

MRI is commonly useful for evaluating these lesions because it can demonstrate changes within the subchondral bone and surrounding joint structures. Treatment depends on the underlying cause and may involve activity modification, physical therapy, pain management, or treatment of the associated cartilage or joint condition.

Physiological Mechanisms of Endometriosis and Pelvic Anatomy;

Endometriosis is a chronic inflammatory condition characterized by the presence of functional endometrial tissue outside the inner lining of the uterus. As illustrated, these aberrant endometrial implants can attach to various pelvic structures, including the ovaries, the outer surface of the uterus, and adjacent peritoneal surfaces.
Under the influence of cyclical ovarian hormones, these ectopic tissue patches undergo similar proliferative and bleeding changes during the menstrual cycle, though the blood has no exit path from the pelvic cavity.

This trapped blood and subsequent local tissue irritation trigger chronic inflammation, leading to the formation of dense fibrous bands known as adhesions. These adhesions can distort normal pelvic anatomy, tethering organs such as the ovaries and Fallopian tubes into fixed, abnormal positions and causing significant pelvic pain, organ dysfunction, or potential fertility challenges.
