Right-sided Endometrioma is a well-known diagnostic trap because it can look very similar to Appendicitis, especially in emergency settings. Here’s how the confusion happens—and how to separate them clinically:
Why the confusion occurs
Both cause right lower abdominal pain
Both may show tenderness in the right iliac fossa
Nausea/vomiting can be present in both
Mild leukocytosis can occur in both
Key differences (very important)
1. Nature of pain
Endometrioma / Endometriosis
Cyclical (worse during periods)
Recurrent episodes
Dull, deep, dragging pain
Appendicitis
Sudden onset
Progressive, continuous pain
Starts around umbilicus → shifts to right lower abdomen
