Some victories in medicine are more than just successful surgeries—they are a testament to teamwork, planning, and trust.
A 44-year-old presented with a severely comminuted proximal humerus fracture. What made this case particularly challenging was his high anaesthetic and surgical risk:
• Previous Pulmonary Thromboembolism (on anticoagulant therapy)
• Uncontrolled hypertension – BP 220/110 mmHg
• SpO₂ 84% on room air
• Obesity with short neck and mouth opening of only 2 fingers, indicating a potentially difficult airway.
After meticulous pre-operative evaluation, optimization, and detailed counselling, our multidisciplinary team successfully performed the surgery under ultrasound-guided interscalene and supraclavicular brachial plexus blocks, avoiding general anaesthesia. ORIF with proximal humerus locking plate fixation was performed, achieving excellent anatomical reduction.
Successfully managing such a high-risk case in a small town is what truly makes a difference. It proves that with the right expertise, advanced technology, and a dedicated team, complex surgeries can be safely performed close to home—without patients having to travel to a big city.
