Blunt abdominal trauma can produce injury patterns that challenge even established management principles.

This case report describes an exceptionally rare hepatic injury where mechanism, anatomy and vascular viability changed the surgical decision-making—and ultimately made a seemingly catastrophic injury salvageable.
