Can light help the brain?

It sounds futuristic, but photobiomodulation — also called transcranial PBM — is a real, non-invasive approach being studied for brain health. It uses gentle near-infrared and red light applied to the head. There's no medication, no electrical current, and no pain — just light.

The idea: certain wavelengths of light are absorbed by your cells' mitochondria (their energy engines), which may support healthy cellular energy, blood flow, and a calmer inflammatory state in the brain. Researchers are exploring it for mood, focus, mental fatigue, and cognitive wellbeing.

By the time most patients come into my office and say "nothing is working," they've been trying for years. Two SSRIs, maybe an SNRI, a round of bupropion, therapy that helped at first and then plateaued — and a version of themselves they remember being that now feels far away.
If that's where you or someone you love is, the first thing worth knowing is this isn't unusual. The largest real-world study of antidepressant treatment we have — STAR*D — found that only about 30% of patients reach remission on their first antidepressant. By the fourth structured step, two-thirds get well. But only by changing the question — not just the prescription.
So I wrote the honest decision tree I walk through with patients in this situation. Re-examining what's actually being treated (bipolar features, trauma, ADHD, medical contributors all hide here more often than people realize). Optimizing what's already in place before reaching for something new. And when the move is genuinely to escalate, switching mechanism, not just molecule — TMS, ketamine, ECT, MAOIs — because failing one system doesn't predict failing another.
Failure of multiple antidepressants is not a verdict. It's a sorting problem. The next move depends on what is actually driving your depression — and that question is almost always answerable.
