Important take home points of “Difficult to Manage Spondyloarthritis “

1) Combination of Biologicals / targeted Synthetic DMARDS - talk of meetings and Panel now.
2) Look and treat comorbidities.
3) Pay special attention to mental health
4) STOP - REASSESS- try combination therapy
5) Phyisotherapy has no substitute

It’s a recent addition to the list.

Had a wonderful and interactive session on “ Scleroderma Renal Crisis” , Organised by Sir Ganga Ram Hospital .

Extremely thankful to Dr Ved Chaturvedi , Dr Neeraj Jain , Dr Manjari and Dr Lalit Duggal .

Important points to remember:
1) Prophylactic use of ACE inhibitors is not warranted
2) At the time of crisis : ACE inhibitors, CCB and sometimes plasmapheresis help..
3) Use of Eculizumab may be warranted in some .

Disussed about POTS ( Postural Orthostatic Tachycardia Syndrome) .
# Recent increase post COVID
# Symptoms mimic Various Rheumatological disorders
# esp Raynauds , Rashes, fatigue, myalgia
# Raised autoantibody in low titres though

So think twice before labelling or diagnosing a pt based simply on autoantibodies.

It was an honour to be part of WRF 2026, World Rheumatology Forum, Raipur.

Extremely thankful to Dr Arun Kedia, Dr Sham and Dr Vinod Ravindran for making me a part of this wonderful meeting .

Got an opportunity to judge unusual cases .

1) Choosing the right biological agent in case of underlying colon inflammation
2) One autoimmune disease can change into other other autoimmune disorder over a period . Always keep reviewing the diagnosis in case of suboptimal response .
3) TB should never be missed

Going back to the place , ( University college of Medical Sciences , GTB, Shahadra, )where once I went as a student to give my final MD Medicine exams in 2011, was pure nostalgia.

Extremely delighted and honoured to be invited as a speaker .

And talk about “ Emerging therapies in GOUT”. In annual Delhi Rheumatology Association CME…

Extremely thankful to Dr Sundeep Upadhyaya and Dr Neeraj Jain for this wonderful opportunity where I read and tried my best to convey few useful points .

# Newer Uricosuric drugs like Dotinurad and Verinurad ( URAT1 inhibitor ) seem useful esp in following scenarios
1) In CKD state 3 and 4 , where other drugs are contraindicated.
2) Improve cardio metabolic functions ( by not inhibiting ABCG2 transporter, which is partially inhibited by febuxostat) . ABCG2 has role in excreting Indoxyl compunds which if accumulated in blood leads to risk of cardiovascular events.
3) Anti inflammatory property
4) Approved in Japan in 2020, and China in 2025
5) Available in India only for research
