"When a Liver Abscess Reaches the Lung
& Destroys a Lobe"
~HepatoBronchial Fistula

A rare and challenging case.

A 60-year-old gentleman with diabetes developed transdiaphragmatic rupture of a liver abscess into the right middle lobe, resulting in a hepatobronchial fistula with destructive lung abscess.

The liver abscess was initially drained by our Interventional Radiology team. However, with irreversible destruction of the middle lobe, we proceeded with right middle lobectomy and disconnection of the fistulous tract.

The surgery was made more challenging by dense pleural adhesions from a previous tubercular pleural effusion a decade earlier.

Pulmonary lobectomy for this complication is exceptionally rare, with only isolated cases described in the literature.

A fascinating reminder of how disease can sometimes cross anatomical boundaries—and why multidisciplinary care matters.
