The truth is, there is no one-size-fits-all answer. The safest delivery method depends on several factors, including the mother's health, the baby's position, pregnancy complications, and your doctor's medical assessment.

Most couples labeled "unexplained" were never given a full investigation in the first place.

In a conventional fertility workup, the focus is often narrow. Ovulation. Tubes. Basic semen analysis.

But fertility is a whole body process & the environment your egg, s***m, and uterus develop in matters.

Here are the areas I almost always investigate deeper with my clients.

👉🏻Gut health

Hidden gut infections, dysbiosis, and chronic inflammation can disrupt hormone signaling and immune balance needed for implantation.

👉🏻Uterine microbiome

The uterus is not sterile. An imbalanced microbiome or chronic endometritis can interfere with embryo implantation and early development.

👉🏻Iron regulation

Ferritin alone does not tell the whole story. Iron overload or poor regulation can increase oxidative stress that affects egg quality and the uterine environment.

👉🏻Insulin sensitivity

Many women are told their blood sugar is "normal" because glucose or A1c look fine. But fasting insulin, HOMA IR, and triglyceride patterns often reveal metabolic stress that impacts ovulation and implantation.

👉🏻Sperm DNA fragmentation

A basic semen analysis can look "normal" while underlying DNA damage in s***m increases miscarriage risk and lowers embryo quality.

👉🏻Endocrine disruptors

Microplastics, BPA, phthalates, and other environmental chemicals interfere with hormone signaling and mitochondrial function in egg and s***m.

👉🏻Chronic sleep deprivation

Your reproductive hormones depend on circadian rhythm. Poor sleep increases inflammation, cortisol, and insulin resistance.

👉🏻Mineral depletion

Minerals regulate energy production, thyroid signaling, blood sugar, and inflammatory balance. When these are depleted, the body may not feel safe supporting pregnancy.

When these pieces are addressed, subtle shifts in physiology can completely change fertility outcomes.

Pregnancy supplements work best when taken at the right time.
Small timing changes can make a big difference.

1. folic acid-take it in the morning after breakfast.
It helps prevent neural tube defects.
2. Iron works best on an empty stomach, or with something like orange juice, but not with milk, tea, coffee, or calcium. This helps prevent anemia and supports baby's growth.
3. Calcium plus vitamin D should be taken after lunch or dinner with a two-hour gap from iron. This supports bones and baby development.
4. DHA (omega-3) after meals for baby's brain and eye development.
5. Vitamin B12, if prescribed, take in the morning, with or without food.

Pregnancy में कौन-कौन से Ultrasound Scans ज़रूरी होते हैं? 🤰👶 | Part 1

Pregnancy के अलग-अलग stages पर ultrasound scans baby की growth और development को assess करने में मदद करते हैं. Dr. Anshu Chamoli, Consultant Gynecologist & IVF Specialist, explains some important scans you may be advised during pregnancy.

🗓️ Scans Covered in Part 1:

1️⃣ Early Pregnancy / Dating Scan (around 6–9 weeks)
Helps confirm the location of the pregnancy, assess viability/heartbeat when appropriate for gestational age, and estimate gestational age.

2️⃣ NT / NT-NB Scan (around 11–13+6 weeks)
Measures nuchal translucency and may assess markers such as the nasal bone. It helps screen for the risk of certain chromosomal conditions and may be combined with blood screening as advised by your doctor.

3️⃣ Anomaly / Level 2 Scan (around 18–22 weeks)
A detailed ultrasound that assesses the baby's anatomy, including major organs and structures, and checks fetal development.

📌 The exact number and timing of scans can vary depending on your pregnancy and your doctor's advice.

Drugs contraindicated in pregnancy

1. isotretinoin : severe congenital malformations
2. Thalidomide: limb defects
3. Methotrexate: miscarriage and anomalies
4. Warfarin: nasal hypoplasia and bleeding
5. ACE inhibitors and ARBs: renal failure, oligohydramnios.
6. Statins are avoided because of fetal developmental risk.
7. Valproate, carbamazepine, phenytoin- neural tube defects
8. Lithium- Ebstein anomaly.
9. Tetracyclines- teeth discoloration, bone defects 10. Aminoglycosides like streptomycin or gentamicin: ototoxicity.
11. Fluoroquinolones: cartilage toxicity
12.Trimethoprim high dose fluconazole: craniofacial and skeletal anomalies.
13. Misoprostol: abortion
