🫁 Early Signs of Lung Transplant Rejection — What Should You Watch For?

After a lung transplant, rejection is an important complication that requires close monitoring. Recognising changes early and informing the transplant team promptly can help with timely evaluation and treatment.

Some warning signs may include:

🔹 New or increasing breathlessness — one of the most important symptoms to watch for
🔹 Persistent or new cough
🔹 Fever
🔹 Unusual weakness or fatigue
🔹 A decline in exercise tolerance or overall functional capacity
🔹 New haziness or changes on a chest X-ray, sometimes associated with fluid or inflammation

However, these symptoms do not automatically mean rejection. Infection can produce similar symptoms, so doctors often need to differentiate between acute rejection, infection and other post-transplant complications.

Depending on the clinical findings and investigations, the transplant team may modify immunosuppressive treatment or undertake further testing to establish the cause.

🚨 Important

After lung transplantation, any new breathlessness, cough, fever or unexplained decline should be reported promptly to the transplant team. Do not wait for symptoms to become severe.

🫁 How Is Interstitial Lung Disease & Pulmonary Fibrosis Diagnosed?

How do we identify Interstitial Lung Disease (ILD) and pulmonary fibrosis?

Diagnosis begins with something very basic—but extremely important: a careful clinical examination.

When a patient presents with a relevant history, listening carefully with a stethoscope can provide an important clue. A characteristic sound called “Velcro crackles” may be heard, particularly at the bases of the lungs.

During evaluation, doctors may also look for signs such as:

🔹 Clubbing of the fingers
🔹 Cyanosis
🔹 Characteristic lung sounds on auscultation
🔹 Changes in exercise tolerance or breathlessness

To understand the disease more thoroughly, investigations may include:

🩻 Chest X-ray
🫁 HRCT Chest
📊 Pulmonary Function Tests (PFTs)
🚶 Six-Minute Walk Test (6MWT)
🧪 Relevant blood investigations

Together, these assessments help determine how extensively the lungs are affected anatomically and physiologically, which supports the treatment and monitoring plan.

🫁 What is Interstitial Lung Disease (ILD)?

Interstitial Lung Disease (ILD) is a group of conditions that can cause scarring and fibrosis of the lungs.

Normally, the lungs are soft and spongy, allowing us to breathe comfortably. With lung fibrosis, the lung tissue becomes thicker and less flexible, and some areas may collapse. As a result, breathing can require more effort.

🔹 How does lung fibrosis develop?
It may develop slowly over several years, or in some cases, progress more rapidly over months.

🔹 What causes ILD?
Sometimes the cause remains unknown — this is called Idiopathic Pulmonary Fibrosis (IPF).

ILD can also occur in association with systemic conditions such as:
• Rheumatoid arthritis
• Scleroderma
• Sarcoidosis and other conditions

Because ILD includes several different diseases, identifying the specific type and cause is important for appropriate evaluation and management.

🌱 Soil, Water & Food: The Pollution We May Not See

At the Good Air Summit & Awards, Dr. M. S. Kanwar highlighted an important dimension of environmental health: pollution monitoring should extend beyond the air we breathe.

Soil and water quality also matter. Regular, location-specific monitoring can help identify environmental contaminants and understand what may be entering our food systems.

Dr. Kanwar emphasized the need for:

🌱 Regular soil-quality assessment
💧 Monitoring of water used for agriculture and household purposes
🥬 Better understanding of environmental contaminants in food production
🏭 Responsible planning of industrial zones
🔬 Stronger investment in indigenous R&D and scientific innovation
🇮🇳 Long-term, sustained efforts rather than short-term initiatives

The conversation ultimately comes back to one question:

Are we looking at environmental health broadly enough—from air and water to soil, food and the systems that produce them?

Creating healthier communities requires science, monitoring, innovation and sustained action at multiple levels.

🌍 Good Air Summit & Awards | Dr. M. S. Kanwar

At the Good Air Summit & Awards, Dr. M. S. Kanwar shared his perspective as a panelist on an important but often overlooked aspect of environmental health:

Pollution is not only an air-quality issue.

Environmental contaminants can affect the air we breathe, the water we use, the soil in which food is grown, and ultimately the wider ecosystem around us. This makes environmental monitoring and prevention a much broader public-health challenge.

Dr. Kanwar also highlighted the importance of moving beyond individual responsibility towards research, innovation and sustainable solutions.

🔬 Greater investment in R&D and indigenous innovation can help develop cleaner technologies, safer industrial processes, improved pollution-control systems, and better methods for monitoring air, water and soil quality.

🌱 Sustainable environmental health requires more than asking individuals to change their behaviour. It also requires responsible products, cleaner systems, scientific research and long-term investment.

🫁 Endobronchial TB: When a Lung Lesion Can Be Misleading

A lung growth or airway obstruction may raise concern for a tumour or another abnormal lesion—but sometimes, the underlying cause can be tuberculosis.

In this case, bronchoscopy and tissue biopsy were performed for a definitive diagnosis. Pathology showed granulomatous changes, and the AFB (acid-fast bacilli) stain was positive, confirming tuberculosis.

The airway growth and obstruction were contributing to the abnormal sounds. After appropriate anti-tubercular treatment and management of the airway swelling:

✅ The abnormal sounds resolved
✅ Chest X-ray showed significant improvement
✅ The previous lung shadow almost disappeared
✅ The left upper lobe became much more open
✅ The patient is clinically doing very well

A sleep study is also planned to determine whether sleep apnea is present independently.

The patient will continue tuberculosis treatment, with a follow-up CT scan after the planned treatment period to assess the lungs and determine whether further treatment is required.

🔎 The Key Lesson

A missed or delayed diagnosis can allow airway obstruction and lung damage to progress. In this case, timely bronchoscopy, biopsy and appropriate treatment helped establish the diagnosis and guide management.

Early diagnosis + correct diagnosis + appropriate treatment can make a significant difference.

🫁 Endobronchial Tuberculosis: Why Bronchoscopy & Biopsy Matter

In this bronchoscopy video, Dr. M. S. Kanwar demonstrates an abnormal lesion inside the airway.

On bronchoscopy, the appearance of such a lesion can raise different possibilities — including a tumour, granuloma, infection or another inflammatory lesion. Visual appearance alone cannot establish the final diagnosis.

Therefore, tissue was taken from the lesion during bronchoscopy and sent for histopathological examination.

🔬 Bronchoscopy helped identify and assess the lesion.
🧪 Biopsy provided the confirmation — Endobronchial Tuberculosis (TB).

This case highlights the importance of obtaining tissue when an unexplained endobronchial lesion is found. A condition that may initially appear suspicious for a tumour can sometimes have an entirely different cause, such as tuberculosis.

The correct diagnosis determines the correct treatment.

🫁 Case Study: Endobronchial Tuberculosis Diagnosed by Bronchoscopy & Biopsy

A young man with a history of anxiety and depression later developed breathing-related symptoms, along with symptoms suggestive of sleep-disordered breathing and snoring. He was treated for suspected sleep apnea with CPAP, which he used intermittently over the next few years.

When he consulted Dr. M. S. Kanwar approximately three months ago, imaging revealed a new suspicious shadow in the left upper lobe.

Because of this new lung lesion, obtaining a tissue/sample diagnosis was important. A bronchoscopy with bronchial lavage and biopsy was performed.

The bronchoscopic evaluation and biopsy ultimately confirmed endobronchial tuberculosis (TB).

🔍 Key Learning

Not every respiratory symptom is explained by sleep apnea alone. When a new or unexplained lung lesion appears, further investigation may be necessary.

In selected cases, bronchoscopy with lavage and biopsy can provide the definitive diagnosis, allowing appropriate treatment to begin without unnecessary delay.

The right diagnosis is the first step toward the right treatment.

This case is shared for medical education and awareness. Diagnosis and treatment should always be guided by a qualified specialist.

Complete Recovery After Tuberculosis Treatment

Following the diagnosis of pulmonary tuberculosis and completion of the prescribed anti-TB treatment, the patient continued to be monitored clinically and radiologically.

TB treatment was stopped in April. Three months later, the follow-up chest X-ray showed near-complete radiological resolution of the previous lung disease.

The previously enlarged lymph node had also reduced significantly and was now approximately 6–8 mm, within the normal range. Only a small residual fibrotic band remained as a healed/residual lesion.

Most importantly:

✅ Chest X-ray — essentially normal
✅ No current respiratory symptoms
✅ Significant resolution of previous lung involvement
✅ Previously enlarged lymph node — almost completely resolved
✅ No ongoing TB treatment required

This case highlights that even extensive pulmonary TB can show remarkable recovery with timely diagnosis, appropriate treatment and adequate follow-up.

The patient was advised that routine TB-specific follow-up was no longer necessary, but to seek medical advice if new respiratory or other concerning symptoms develop in the future.

Early diagnosis. Complete treatment. Appropriate follow-up. Better outcomes.

This case is shared for medical education and awareness. Individual treatment and follow-up should always be determined by the treating physician.
