Just as in other regions of the body, physiotherapy hastens ligament healing and relieves pain so that you can enjoy life again as soon as possible. Bulging Discs


A bulging disc injury is a common spine injury sustained to your spine's intervertebral disc. Spinal discs are the shock-absorbing rings of fibrocartilage and glycoprotein that separate your bony vertebral bodies, while allowing movement at each spinal level, and enough room for the major spinal nerves to exit from the spinal canal and travel to your limbs. The annulus is the outer section of the spinal disc, consisting of several layers of multi-directional fibrocartilaginous fibres all densely packed to create a wall around the glycoprotein filled jelly-like disc nucleus. A disc bulge (commonly referred to as slipped disc), can potentially press against or irritate the nerve where it exits from the spine. This nerve pinch can cause back pain, spasms, cramping, numbness, pins and needles, or pain into your legs. Bone Injuries

While lower back pain treatment will vary depending on your specific diagnosis, your physiotherapist will have the following aims. PHASE I - Back Pain Relief & Protection

Managing your back pain is the main reason that you seek treatment for lower back pain. In truth, it was actually the final symptom that you developed and should be the first symptom to improve. Your physiotherapist will use an array of treatment tools to reduce your pain and inflammation. These include: ice, electrotherapy, acupuncture, de-loading taping techniques, soft tissue massage. A course of non-steroidal anti-inflammatory drugs such as ibuprofen may also help in this phase. PHASE II - Restoring Normal ROM and Strength. Early Back Exercises. As your lower back pain and inflammation settles, your lower back pain may feel better but you are actually more vulnerable for re-injury during this honeymoon period when you don’t have pain but your muscles and ligaments are weak. During this phase your physiotherapist will turn their attention to restoring your normal lumbar spine motion, muscle length and resting tension, muscle strength and endurance, proprioception, balance and gait (walking pattern). PHASE III - Restoring Full Function

Depending on your chosen work, sport or activities of daily living, your physiotherapist will aim to restore your back’s function to safely allow you to return to your desired activities. Clinical Bottom Line Score
This rule can be used to help screen patients with lower-extremity symptoms for lumbar spinal stenosis as defined by an expert panel. A score of greater than seven indicates a small but perhaps meaningful shift in probability for LSS and a score of less than or equal to two produces a moderate shift in probability that the individual does not have LSS. This rule has been validated and may be applied to practice within the confines of the study parameters; however, further external, broad validation studies still need to be performed.

Foot Alignment - A hidden cause of Back pain.

Pronation This is the name given to the natural inward roll of the foot as the arch of the foot collapses during walking.

This occurs between touching down with the heel and lifting off with the toes, and it is a natural shock absorption. It is not 100 per cent effective, however, and some of the shock of each step will be transmitted up the legs to the pelvis and thence to the spine.

Both under-pronation and over-pronation can cause problems. Under-pronators do not have enough roll, so they do not effectively shock-absorb.

They usually have high arches and as a result they may get knee pain and low back pain. They need flexible shoes with good cushioning.

Over-pronators have too much roll and the foot collapses on each step. They get strain on the whole of the lower body and may have persistent low back pain. They generally have fairly at arches and need some sort of support in their shoes.

A medial arch support can be inserted into the shoe which may help over-pronation.

Choose the right bed
When you consider that you probably spend a third of your life in bed, it makes sense to make that time as comfortable as possible and as beneficial to your back as you can.

The position you sleep in
Do you move around a lot in bed?

Some people seem to do so naturally, changing their position frequently throughout their sleep time.

Some who do are dimly conscious that they do so because they do not feel comfortable. If you are in the latter category then you may find that you wake feeling stiff back and with an ache in your back.

People who sleep on their back are most likely to wake feeling uncomfortable. This is because lying on the back with your legs straight out tends to throw the back into an exaggerated curve in the low back area.

The best position for the back seems to be lying on your side with the knees bent to about a 45-degree angle. Trying to get into that position is a good idea. Putting a pillow or a small cushion between the knees may make it more comfortable.

If you can’t get used to lying on your side, then try using a pillow or cushion under your knees to create the knee-bend, to take some of the pressure of the lower back. People prone to sciatica usually find this helps.

People who lie on their front may find that trying to sleep on their side is helpful, because they are then more likely to get their legs into a more relaxed position as mentioned above. Lying on the front will straighten the legs.

Don't panic if you have Back pain
If you do experience an episode of back pain, there is no need to panic. Although it can be painful and may restrict your mobility it is highly likely that it will get better of its own accord in a relatively short time. While it is natural to worry in case the pain is due to some serious injury or to some underlying condition, in fact most back pain is not serious at all. The vast majority of back pain will improve within two days to two weeks. Most simple back strains do not cause any lasting damage. It is worth knowing that about half of all the people who experience such an episode of back pain will have another one within two years. The thing is that it can be avoided if you take the correct actions and respect your back.
