Small Lung Nodule in a Patient with Warthin Tumour: Cancer or Not? ๐Ÿค”

A patient with a known Warthin tumour (a salivary gland tumour strongly associated with smoking) and a chronic smoker was incidentally found to have a left upper lobe lung nodule.
โœ… No respiratory symptoms.

Instead of delaying the diagnosis or living with uncertainty, the patient chose to undergo a biopsy.

๐Ÿฆ  BAL culture: Escherichia coli (E. coli)

Diagnosis: Secondary Organising Pneumonia (Not Cancer โœ…)

โ€ข Although Warthin tumour is >99% benign, it is important to evaluate a lung nodule thoroughly, especially in smokers, to rule out malignancy.

โ€ข A diagnosis of Warthin tumour may serve as an opportunity to consider lung cancer screening in appropriate smokers.

A 26-year-old female presented with marked jaundince

chest X-ray rapidly worsened to an ARDS-like picture.
And required high oxygen support

๐Ÿ”น CT Chest: Bilateral ground-glass opacities, interstitial thickening and significant mediastinal lymphadenopathy.
๐Ÿ”น CT Abdomen: Hepatosplenomegaly with no significant focal hepatic abnormality.

? Tuberculosis (ruled out as bal genexpert and Igra was negative)
?lymphoma
?sarcoidosis

EBUS-guided mediastinal lymph node cryobiopsy was performed.

The initial histopathology sections were inconclusive.
After discussion with our pathologist, the deeper sections revealed ill-defined granulomas.
The diagnosis of ๐’๐€๐‘๐‚๐Ž๐ˆ๐ƒ๐Ž๐’๐ˆ๐’ was made

The patient showed response to steroids and was eventually discharged on room air. ๐Ÿ™Œ

๐Ÿฆท TOOTH IN THE LUNG! ๐Ÿซ

A tooth was found lodged in the right bronchus, most likely aspirated during intubation. The diagnosis was confirmed on Chest X-ray, and it was successfully removed using a cryoprobe.

Learning Points & Challenges:

โœจ Don't miss it on the Chest X-ray โ€” a tooth can be easily overlooked.

โ„๏ธ The tooth was large, hard, and dry, making cryoadhesion difficult. Adequate saline instillation was achieved for a good freeze.
