Safe entry in laparoscopy
Safe entry in laparoscopy is often underestimated.
In reality, it is the most critical step of the entire procedure.

Inside the abdomen, everything is under vision.
But the initial entry?

👉 It is blind.

And that is where most complications occur:
• Bowel injury
• Major vessel injury
Before entering, a simple pause can prevent serious complications.

Ask yourself:
✔ Is the abdomen adequately lifted?
✔ Is the angle correct?
✔ Are there previous scars or adhesions?
✔ Do I need an alternative entry site?
✔ Is the anatomy normal or altered?
✔ Is the bladder emptied?

There is no single “safest” technique.
Each patient demands the right approach and sound judgment.

👉 In the end, safety is not just technique — it is decision-making.

Pneumoperitoneum creates the working space for laparoscopy using CO₂. But it also affects physiology. Increased intra-abdominal pressure elevates the diaphragm, reduces lung expansion, decreases venous return, and raises CO₂ absorption. Maintaining safe pressure (12–15 mmHg) is essential. During laparoscopy, vision is everything.

If the image suddenly fogs, don’t panic — and don’t continue blindly.
Pause. Check the laparoscope tip.

Causes of laparoscopic lens fogging:

1️⃣ Temperature difference (MOST COMMON)
• Cold laparoscope introduced into warm abdomen
• Condensation forms on lens surface
• Primary mechanism of fogging

2️⃣ High intra-abdominal humidity
• CO₂ insufflation + body fluids create a humid environment
• Moist air condenses on the lens
• Especially in long surgeries

3️⃣ Inadequate warming of the scope
• Scope kept on trolley / AC room
• Not pre-warmed before insertion
• Leads to repeated fogging even after cleaning

4️⃣ Repeated insertion–withdrawal of scope
• Scope moves between environments
• Temperature fluctuation → condensation again

5️⃣ Anti-fog solution not used properly
• Lens cleaned but left dry
• No surfactant layer
• Water droplets reform quickly

6️⃣ Lens micro-scratches or residue
• Old scope, detergent residue, scratches
• Water spreads irregularly → persistent haze (looks like fog)

Clear vision isn’t a luxury — it’s a safety requirement.
