Case of the Week | Total Knee Replacement (Right Knee)

Patient: 55 year-old patient

Preoperative Presentation

• Persistent right knee pain for several years.
• Difficulty walking, climbing stairs, and performing daily activities.
• Failure of conservative treatment (medications, physiotherapy, activity modification).
• Clinical examination showed varus deformity with 5–7° fixed flexion deformity (FFD).
• X-rays revealed advanced medial compartment osteoarthritis with significant joint space narrowing, osteophyte formation, and subchondral sclerosis.

Surgical Procedure

✔ Right Cemented Total Knee Arthroplasty (TKA)

The decision for Total Knee Replacement is based on the patient’s pain, functional disability, clinical examination, and radiographic findings—not on X-ray appearance alone. The aim is to restore mobility, relieve pain, and improve quality of life when conservative treatment no longer provides adequate relief.

Recurrent Patellar Dislocation – Treated with MPFL Reconstruction

✅ Recurrent lateral patellar instability is most commonly due to injury of the medial patellofemoral ligament (MPFL), the primary restraint to lateral patellar translation from 0–30° of knee flexion.

Indications for isolated MPFL reconstruction:
• Recurrent patellar dislocation
• No high-grade trochlear dysplasia
• TT–TG distance within acceptable limits
• Patella alta not significant
• Correctable without bony procedures

Goals of surgery:
✔ Restore medial patellar stability
✔ Prevent recurrent dislocation
✔ Preserve normal patellar tracking
✔ Enable return to daily activities and sports
