Bowel endometriosis is a form of deep endometriosis where endometriotic lesions affect the intestine, commonly involving the sigmoid colon.

In this case, the intestine was completely adherent near the uterus, with a deep endometriotic nodule involving the bowel. The patient was experiencing severe pain during periods along with significant difficulty during bowel movements.

Depending on how deeply the lesion involves the bowel, surgical management may involve discoid excision or bowel resection with anastomosis.

In this case, the lesion required resection and anastomosis, where the affected segment of bowel was removed and the healthy ends were reconnected laparoscopically.

The final laparoscopic view shows the bowel after completion of the procedure.

⚠️ Severe period pain or bowel-related symptoms during menstruation should not be ignored. If endometriosis is suspected, proper evaluation by an experienced specialist is important.

Low AMH doesn’t just mean fewer eggs—it means your IVF strategy needs careful planning.

Sometimes, a patient with low AMH undergoes IVF, retrieves only 1–2 eggs, creates embryos, transfers them, and after a failed cycle, stops treatment or returns years later.

In selected patients, instead of rushing to embryo transfer, doctors may consider multiple stimulation cycles to collect additional eggs and create more embryos for freezing. This can be particularly relevant when the patient wants to preserve the possibility of having more than one child in the future.

The goal is not simply to ask, “How many eggs did we get this cycle?” The bigger question is: “What are your future fertility goals, and how can we preserve your reproductive potential?”

Low AMH requires an individualized IVF strategy based on age, ovarian reserve, previous response to stimulation, embryo development and future family plans.

An “open” fallopian tube on HSG does not always mean the tube is completely normal.

In this hysteroscopy, we could see a significant adhesion band near the opening of the patient’s left fallopian tube. The adhesion was partially hiding and narrowing the tubal opening.

During an HSG, dye may pass around an adhesion and make the tube appear open, even though there may still be an underlying structural problem.

In this procedure, the adhesion band was carefully cut, allowing us to visualize the tubal opening more clearly. Further assessment also showed significant narrowing/blockage of the tube.

This is why, in selected infertility cases, hysteroscopy can provide valuable direct information about the uterine cavity and tubal openings that may not be fully appreciated on an HSG alone.

When the s***m count is extremely low, especially in conditions such as cryptozoos***mia, natural fertilization can become difficult.

This is where ICSI (Intracytoplasmic S***m Injection) can play an important role.

During ICSI, an embryologist carefully selects a s***m and injects it directly into the egg using a very fine needle under a microscope. 🔬🧬

In this video, we show the ICSI procedure and explain how directly injecting a s***m into the egg can help overcome the fertilization barrier in selected cases of severe male-factor infertility.

Once fertilization occurs, the fertilized egg can develop into an embryo, which may then be considered for transfer as part of the IVF process.

AI vs Embryologist: Who Should You Trust?

AI is becoming increasingly common in IVF laboratories, including for embryo assessment and grading. But does having an AI-based system automatically mean that your embryo is being assessed better?

AI models are trained using large datasets of embryo images and their associated outcomes. However, embryo assessment is complex. Embryos have three-dimensional structures and can change dynamically over time.

An experienced embryologist can assess the embryo in real time and combine the visual findings with their training, experience and clinical context.

AI can be a useful supporting technology, but it should not replace experienced human expertise.

The best IVF laboratory is not necessarily the one with the fanciest technology. Look for a centre where advanced technology and experienced clinicians and embryologists work together.

Artificial Intelligence is powerful — but it works best when combined with Natural Intelligence.

MicroTESE (Microsurgical Testicular S***m Extraction) is a specialized s***m-retrieval procedure used in selected men with azoos***mia.

During the procedure, a surgical microscope is used to identify areas of the testicular tissue that may contain s***m. The tissue is then carefully examined by the embryology team to search for viable s***m.

If s***m are successfully retrieved, they may be used for ICSI as part of IVF treatment.

Zero s***m in the semen does not always mean zero s***m in the testicular tissue. However, MicroTESE is not suitable for every patient, so proper evaluation by a male fertility specialist is important.

Open Fallopian Tubes But Still Not Pregnant?

A normal HSG does not always tell the complete story when it comes to infertility.

In this patient, laparoscopy revealed severe pelvic adhesions. The intestines, o***y, Fallopian Tubes and surrounding pelvic structures were extensively stuck together. Even the ureter was trapped within the adhesions.

Through laparoscopic adhesiolysis, these adhesions were carefully released. After that, a Methylene Blue Dye Test showed dye spilling through both Fallopian Tubes.

This highlights an important point:

“My tubes are open, so why am I not getting pregnant?”

Because having an open tube is not the only factor that matters. Severe adhesions around the tubes, ovaries and pelvic structures can also affect fertility and may not always be clearly identified on HSG or Sonography.

If you have been trying to conceive without success, the right diagnostic approach should be decided based on your individual case by a fertility specialist.

IVF trigger is an important step that helps prepare the follicles for egg retrieval. But what happens when eggs are immature or when no eggs are retrieved despite having multiple follicles?

This is where the concept of a Dual Trigger comes in — combining an HCG trigger with an Agonist trigger.

The agonist trigger helps induce the body’s natural LH surge, while HCG provides a longer LH-like effect. The goal is to improve follicular maturation and increase the chances of retrieving mature MII (M2) eggs.

For patients with previous issues such as poor egg maturity or empty follicle syndrome, the trigger strategy may be an important part of the IVF plan.

And yes, a Dual Trigger does not automatically mean that fresh embryo transfer is impossible. Fresh transfer can still be considered in appropriate cases based on the overall clinical situation.
