A diagnosis of necrozoos***mia does not automatically mean you need donor s***m.

Necrozoos***mia is a condition in which all or nearly all s***m found in a semen sample are dead. But before giving up on biological parenthood, further evaluation may help determine whether viable s***m can be identified.

In selected cases, specialists may consider advanced s***m viability testing, surgical s***m retrieval procedures such as MicroTESE, and ICSI (Intracytoplasmic S***m Injection), depending on the findings and individual circumstances.

Immotile s***m are not always dead s***m. Distinguishing between live and dead s***m is important, and specialized techniques may help identify viable s***m for treatment.

Every case is different, and success cannot be guaranteed. A thorough evaluation by a male fertility specialist and reproductive team is essential to understand the available options.

S***m Found After TESA! 🔬 The Search Under the Microscope

What happens after TESA? The search for s***m begins under the microscope!

TESA (Testicular S***m Aspiration) is a procedure in which a sample is collected directly from the testicular tissue using a fine needle. The collected sample is then carefully examined in the embryology laboratory by a trained embryologist to identify s***m.

In men with azoos***mia (zero s***m count in the ej*****te), s***m may sometimes still be present in the testicles, depending on the underlying cause. When s***m are successfully retrieved, they may be used for ICSI (Intracytoplasmic S***m Injection), where a single s***m is injected directly into an egg during IVF treatment.

Every sample requires careful examination, and s***m retrieval is not guaranteed in every case. The appropriate procedure depends on the cause of azoos***mia and the patient’s individual evaluation.

Every s***m identified under the microscope represents a possibility for couples hoping to achieve parenthood. ❤️

Even when an IVF cycle goes perfectly and a good-quality embryo is transferred, pregnancy is not guaranteed.

Embryo implantation is a complex process influenced by multiple factors. Embryo morphology, chromosomal status, uterine factors, and other biological variables can all play a role. Even a euploid embryo cannot guarantee 100% success.

That is why a failed IVF cycle does not automatically mean the embryo transfer was performed incorrectly or that the patient did something wrong. Bed rest alone cannot guarantee implantation either.

IVF is a journey that requires realistic expectations, proper counselling, and clear communication between the doctor and the patient. Some patients conceive in their first attempt, while others may need additional attempts depending on their individual circumstances.

Understanding these possibilities before starting treatment can help patients make informed decisions and approach IVF with realistic expectations.

Planning laparoscopic surgery for a chocolate cyst or endometriosis? There are a few important things you should know before choosing your surgeon.

Endometriosis surgery is different from routine laparoscopic surgery. Simply draining a chocolate cyst may not adequately treat the underlying disease if other endometriosis lesions are present.

Depending on the extent of the disease, surgery may involve carefully excising visible endometriosis lesions while protecting important structures such as the ovaries, bowel and ureters.

The surgeon’s experience in treating endometriosis is important because the goal is to manage the disease effectively while minimising surgical risks and preserving fertility wherever possible.

Before choosing a centre, ask about the surgeon’s experience with complex endometriosis surgery, the proposed surgical approach, and the potential benefits and risks in your specific case.

The right treatment plan starts with the right diagnosis and an experienced endometriosis specialist.

Can a mosaic embryo lead to a healthy pregnancy? Yes, it can — but understanding the science behind it is important.

After PGT-A testing, embryos may be reported as euploid (chromosomally normal), aneuploid (chromosomally abnormal), or mosaic (showing a mixture of cells with different chromosome results).

These are three distinct categories, and they should not be treated as interchangeable.

Some mosaic embryos, particularly those with lower levels of mosaicism, may be considered for transfer after careful genetic counselling and specialist evaluation. However, outcomes vary, and mosaic embryo transfer may carry additional risks.

Stories shared on podcasts and social media may leave out important scientific details. One person’s experience should not replace an individualised medical decision.

Before deciding against embryo biopsy or choosing a mosaic embryo for transfer, discuss your PGT-A report, the level and type of mosaicism, and the available options with your fertility specialist and genetic counsellor.

HSG can help assess whether the fallopian tubes are open, but laparoscopy can reveal problems that may not be detected on HSG, including pelvic adhesions, endometriosis, and other abnormalities around the tubes and ovaries.

In this case, the patient had been trying to conceive with ovulation studies but pregnancy was not happening. During laparoscopy, blockages were identified and opened, while significant adhesions caused by previous pelvic inflammation/infection were also found and released.

Both fallopian tubes were then checked and confirmed to be open.

This is why, in appropriately selected patients with unexplained infertility or suspected tubal/pelvic disease, laparoscopy can provide both diagnostic information and an opportunity to treat certain fertility-related problems during the same procedure.

when a regular cerclage may not provide enough support, an abdominal cerclage can be considered in selected cases.

laparoscopic abdominal cerclage allows the stitch to be placed higher around the cervix through a minimally invasive approach.

in this procedure, mersilene tape is carefully placed and securely tied to provide cervical support.

a specialised option for selected women with cervical insufficiency or a history of failed vaginal cerclage.

This patient is 39 years old and has been married for 8 years. She has been trying to conceive for a long time and, like most couples, naturally wishes for a pregnancy.

But at 39, simply waiting longer for a natural pregnancy may not be the best approach. Female age is one of the most important factors affecting fertility. With increasing age, egg quality and embryo quality can decline, which can also reduce IVF success rates.

Many patients think, “If natural pregnancy doesn’t happen, we can always do IVF later.” But IVF is not an age-independent solution. The chances of success can also decrease as reproductive age increases.

If you are over 37 and have been trying for a long time, the important question is not only “When will I get pregnant?” but “What is the right treatment for me, and how soon should I start?”

Every couple is different, so fertility evaluation and an individualized treatment plan are essential.

PRP is increasingly being marketed as “ovarian rejuvenation”, with some patients being led to believe that their AMH can dramatically increase after treatment.

But the reality is different.

Ovarian PRP is currently considered an experimental treatment. Some studies have reported possible improvements in AMH levels or oocyte numbers in certain patients, but this does not mean that an AMH of 0.5 will become 3.5.

More importantly, there is still limited evidence that PRP significantly improves the number of embryos or blastocysts obtained or guarantees a higher chance of pregnancy.

So, PRP should not be considered a magic treatment for low AMH.

If your AMH is very low, don’t delay appropriate fertility treatment for months or years hoping that PRP will “rejuvenate” your ovaries. Understand the limitations, discuss the evidence with a fertility specialist, and have a clear treatment plan.

PRP may be an experimental option — but it is not a miracle cure for low AMH.
