It was a privilege to interact with such an enthusiastic audience during our Public Health Lecture on “How to Keep Your Heart Healthy – Prevention is Better than Cure.”

We discussed practical ways to prevent heart disease, recognize early warning signs, control blood pressure, diabetes and cholesterol, and adopt simple lifestyle habits that protect the heart for life.

Not Every “Significant” Lesion Needs a Stent

We are happy to share an interesting case where the use of latest technology helped us avoid an unnecessary intervention.

A 50-year-old female with known coronary artery disease (multiple prior PTCA), along with comorbid CLD Grade II varices and anemia, was referred to us with angiography suggesting significant ostial RCA stenosis. While the lesion appeared severe on angiography, we chose to go beyond visual assessment and evaluate its true clinical significance.

Intravascular ultrasound (IVUS) revealed an intermediate lesion with ~55% plaque burden, which raised uncertainty about its functional impact. To further clarify, fractional flow reserve (FFR) was performed which measures blood pressure drop across the lesion . It showed a value of 0.88, confirming that the lesion was not hemodynamically significant.

Based on combined anatomical and physiological assessment, the decision was made to defer stenting, thereby avoiding an unnecessary intervention—particularly important in this patient given the added risks of CLD and anemia.

This case reinforces a crucial principle in modern cardiology: angiography alone can be sometimes misleading—physiology guides appropriate management. The integration of IVUS and FFR allows us to practice precision-based, patient-centered care, ensuring better outcomes while minimizing risk.
