Frozen shoulder, also called adhesive capsulitis, involves stiffness and pain in the shoulder joint. The condition is called "frozen" shoulder because the more pain that is felt, the less likely the shoulder will be used. Lack of use causes the shoulder capsule to thicken and becomes tight, making the shoulder even more difficult to move -- it is "frozen" in its position. Signs and symptoms typically begin slowly, then get worse. Over time, symptoms get better, usually within 2 to 3 years.
For some people, the pain worsens at night, sometimes disrupting sleep.
🎈What is Frozen shoulder ?
Frozen shoulder has been defined by the American Academy of Orthopedic Surgeons as "a condition of varying severity characterized by the gradual development of global limitation of active and passive shoulder motion where radiographic findings other than osteopenia are absent." The condition is also characterized by severe shoulder pain.
Frozen shoulder is also referred to as adhesive capsulitis, painful stiff shoulder, and periarthritis.
🎈What is the main cause of frozen shoulder?
Also known as adhesive capsulitis, frozen shoulder is usually caused by inflammation. The capsule of the shoulder joint has ligaments that hold the shoulder bones to each other. When the capsule becomes inflamed, the shoulder bones are unable to move freely in the joint. Shoulder becomes stiff. It is more common in diabetes patients. Some additional medical problems associated with frozen shoulder include hypothyroidism, hyperthyroidism, Parkinson's disease, and cardiac disease.
The shoulder joint is enclosed in a capsule of connective tissue. Frozen shoulder occurs when this capsule thickens and tightens around the shoulder joint, restricting its movement. It's unclear why this happens to some people.
Having to keep a shoulder still for a long period increases the risk of developing frozen shoulder. This might happen after having surgery or breaking an arm.
🎈🎈How is frozen shoulder diagnosed?
To diagnose frozen shoulder, your doctor will:
* Discuss your symptoms and review your medical history.
* Conduct a physical exam of your arms and shoulders.
* X-rays of the shoulder are also routinely obtained to make sure the cause of the symptoms is not due to another problem with the shoulder.
* Advanced imaging tests, such as MRI and USG are usually not needed to diagnose frozen shoulder. They may be taken to look for other problems, such as a rotator cuff tear.

After a period of worsening symptoms, frozen shoulder tends to get better, although full recovery may take up to 3 years. It's unusual for frozen shoulder to recur in the same shoulder. But some people can develop it in the other shoulder, usually within two to three years.
🎈What is hydrodilatation for frozen shoulder?
Another treatment, called "hydrodilatation," or “Hydrotamponade” involves injecting the shoulder with anaesthetic agent and saline to expand the joint, and this often provides effective short-time relief. However, it remains unclear whether relief stems from the anaesthetic agent or dilation of the joint. This is called “ Brisement Effect”.
Treatment involves stretching the shoulder joint capsule and sometimes injecting anaesthetic agent and Ozone gas and supportive medication into the joint capsule. In no case, surgery is needed. Injection is aimed to used to loosen the joint capsule.
🎈🎈What is Manipulation under anesthesia for Stiff shoulder ? Is it recommended technique for Frozen Shoulder ?
It’s not at all recommended; rather it is dangerous.
Manipulation under anesthesia: During this procedure, you are put to sleep. Your doctor will force your shoulder to move which causes the capsule and scar tissue to stretch or tear. This releases the tightening and increases range of motion ; as it was wrongly thought.
🎈🎈What is role of Shoulder arthroscopy in Frozen shoulder ?
Needed very very rarely for calcified capsule or refractory cases.
Shoulder arthroscopy: In this procedure, your doctor will cut through tight portions of the joint capsule. This is done using pencil-sized instruments inserted through small incisions around your shoulder.
🎈🎈 What’s the outlook for frozen shoulder?
Simple treatments, such as use of pain relievers and shoulder exercises, in combination with Dr Sandeep Agrawal technique are often enough to restore full motion and function within two to three months. Even left completely untreated, range of motion and use of the shoulder continue to get better on their own, but often over a slower course of time. Full or nearly full recovery is seen after about two to three years.
