In this episode, Dr. Deepak Rathore, Senior Consultant in Plastic and Reconstructive Surgery at Yatharth Super Speciality Hospital, Greater Noida West, presents a case that tests the limits of what a multidisciplinary oncology team can achieve for a patient who had already been turned away by multiple hospitals.

The patient arrived with cancer of the right oral cavity. This was not a first-time diagnosis. Three years earlier, the same patient had been treated for cancer in the same region and had undergone surgery at that time. The recurrence in the same site, combined with the patient's prior surgical history, created a clinical scenario so complex that several hospitals declined to take the case.

The surgical oncology team first performed the cancer resection, removing the tumor from the oral cavity. What followed was the reconstructive challenge: the surgery had left a significant defect in the oral cavity that needed to be rebuilt in a way that would restore both function and structure.

Dr. Rathore and his plastic surgery team performed a free flap reconstruction. This is a technique in which tissue, including skin, muscle, and blood vessels, is harvested from another part of the patient's own body and transplanted to the site of the defect. The blood vessels are then reconnected under a microscope in a process known as microvascular anastomosis, giving the transplanted tissue its own blood supply in its new location. Free flap reconstruction is currently the most advanced and reliable method available for rebuilding complex defects after oral cancer surgery, particularly in cases involving prior surgery and scarring.

The patient recovered well, was discharged 15 days after surgery, and returned to their home country within a month.

In this episode, Dr. Amit Goel, Senior Director of Kidney Transplant, Uro-Oncology, Robotics, and Urology at Max Hospital, Gurugram, walks us through a complex prostate cancer case from Mauritius that required careful sequencing of treatment before surgery could even begin.

The patient was a 65-year-old man who presented with a serum PSA level of 78 — a marker that, in the context of prostate cancer, signals significant disease activity. A biopsy confirmed prostate adenocarcinoma with a Gleason score of 4+3, placing it in the intermediate-high risk category. A PSMA PET scan, one of the most sensitive imaging tools available for prostate cancer, revealed that the disease had extended beyond the prostate itself and was involving regional lymph nodes, making this a locally advanced case.

Rather than proceeding to surgery immediately, the team first administered androgen deprivation therapy, a treatment that blocks the male hormones that drive prostate cancer growth. The response was significant: the PSA dropped from 78 all the way down to 1.4, indicating that the cancer had been substantially brought under control and the conditions for safe surgical intervention had been established.

The surgery performed was a robotic radical prostatectomy, in which the entire prostate gland was removed using robotic-assisted precision, combined with a bilateral pelvic lymph node dissection to address the regional lymph node involvement on both sides. The procedure was completed without complications, and the patient was discharged on the third day after surgery.

The outcomes speak directly to the quality of the work: no urinary leakage, and a post-operative serum PSA of 0.04, which is effectively undetectable. By removing the cancer in its entirety, the team has significantly reduced the risk of future recurrence-related complications. The patient is recovering well and on his way back to Mauritius.
