Stage 4 Lung Cancer: Is Treatment Still Worthwhile?

A Stage 4 diagnosis does not always mean that treatment options have run out. Depending on the cancer type, molecular profile, extent of spread, symptoms, and overall health, treatment can help control the disease, manage symptoms, and improve quality of life.

In this video, Dr. Parveen Yadav, Chief & Senior Consultant, Thoracic Surgery, explains the role of biopsy, molecular testing, targeted therapy, immunotherapy, chemotherapy, radiotherapy, and selected surgical procedures in Stage 4 Lung Cancer.

Remember: Do not make treatment decisions based on stage alone. Every patient is different, and treatment should be planned after reviewing scans, biopsy, molecular reports, and overall health.

• Physiotherapy recovery

• Bullous Lung Disease

Physiotherapy after chest surgery is important — but in many cases, it can begin even before surgery.

Pre-operative physiotherapy, often called prehabilitation, helps prepare the body and lungs for surgery. Depending on your condition and fitness level, a physiotherapist may guide you through:

• Breathing exercises to improve lung expansion
• Effective coughing techniques to help clear secretions
• Walking and conditioning exercises to improve stamina and mobility

After thoracic surgery, pain and reduced movement can sometimes make breathing shallower and allow mucus or secretions to collect in the chest.

This is why post-operative physiotherapy may include:

• Deep breathing exercises to support lung expansion
• Coughing techniques to keep the airways clear
• Early sitting, standing and walking as advised
• Shoulder and chest mobility exercises to restore movement

The aim is not simply to “exercise” after surgery. It is to help patients prepare better before surgery, regain movement safely afterwards, and support a smoother recovery.

Every patient is different, so the timing and intensity of physiotherapy should be planned according to the type of surgery, lung condition and overall fitness.

Bullous Lung Disease can stay unnoticed for a long time — but certain changes should not be ignored.

A bulla is a large air-filled space that forms when damaged air sacs in the lung merge together. Small bullae may cause no symptoms, but larger ones can compress healthy lung tissue and contribute to increasing breathlessness.

One important complication is rupture of a bulla, which can allow air to collect around the lung and cause a pneumothorax.

Warning symptoms that need prompt medical attention include:
• Sudden one-sided chest pain
• Sudden or worsening breathlessness

Not every bulla needs surgery. Small, symptom-free bullae may only require regular monitoring, while treatment for breathing difficulty can include inhalers, pulmonary rehabilitation, or oxygen when appropriate.

Surgery may be considered when a bulla becomes very large, significantly compresses the functioning lung, causes severe symptoms, or leads to repeated complications.

The key is to notice changes in your breathing and seek timely evaluation rather than waiting for symptoms to become severe.

Why is a proper pre-operative workup important before thoracic or lung surgery?

In this video, Dr. Parveen Yadav, Chief & Senior Consultant – Thoracic Surgery, explains the important evaluations and tests that may be required before surgery.

You’ll learn:
• What doctors assess during history and physical examination
• When CT scan, PET-CT, bronchoscopy, or biopsy may be required
• The role of lung function tests such as Spirometry and DLCO
• When walking tests, stair-climbing assessment, or CPET may be needed
• Why blood tests, ECG, and cardiac evaluation are performed
• How medicines, allergies, and previous anaesthesia problems are reviewed
• Why stopping smoking before surgery is beneficial
• Why diabetes, anaemia, and overall fitness may need optimisation before surgery

Not every patient requires every test. The pre-operative assessment is tailored according to the patient’s condition, planned surgery, lung function, and overall health.

The purpose of pre-operative workup is to identify potential risks early and prepare the patient better for surgery.

Thymoma can be silent — but it should never be ignored.

A thymoma is a tumor arising from the thymus gland, located behind the breastbone. It may cause no symptoms and can sometimes be detected incidentally on a chest X-ray or CT scan.

Possible signs can include:
• Chest heaviness
• Persistent cough or breathlessness
• Swelling of the face or neck
• Drooping eyelid or double vision
• Difficulty swallowing
• Symptoms associated with myasthenia gravis

If a thymic mass is found on a scan, proper evaluation by a thoracic specialist is important to understand its nature, extent, and the appropriate treatment approach.

An incidental finding does not always mean an emergency — but it does deserve expert evaluation.
