FMRI Performs Country’s First Pivot-TR Device Implantation on an International Patient with Advanced Right Heart Failure
GURUGRAM, INDIA — Dr Manjinder Sandhu and Team at Fortis Memorial Research Institute (FMRI) and Prof Kim and team from Pusan, South Korea have successfully performed a groundbreaking transcatheter tricuspid valve intervention on a 35-year-old patient suffering from end-stage right ventricular dysfunction and torrential tricuspid regurgitation (TR).
Ms. Caversenn, a resident of Trinidad and Tobago, presented to FMRI with a history of recurrent hospitalizations for severe right-sided heart failure. Her condition was complicated by refractory lower-limb edema, recurrent ascites requiring multiple paracenteses, hepatic fibrosis, and worsening renal dysfunction. Despite seeking care across numerous medical centers from different countries, her symptoms remained debilitating.
Given her severe right ventricular failure, massive tricuspid annular dilation, and the presence of pre-existing pacemaker leads, standard percutaneous edge-to-edge repair options were unsuitable, and open surgical repair posed a prohibitive risk.
Following a comprehensive evaluation, the multidisciplinary Heart Team at FMRI opted for transcatheter treatment using the novel Pivot-TR Device. The procedure was performed successfully, yielding immediate on-table hemodynamic improvement. Post-procedure, the patient demonstrated significant clinical recovery and relief from her congestive symptoms.
This marks only the third implantation of the Pivot-TR device in the country, offering a promising alternative for high-risk patients with advanced tricuspid valve disease and poor prognosis despite optimal medical therapy.
