*Successful Management of a Highly Complex Twin Pregnancy led by me at Artemis Hospital; Gurgaon as Multidisciplinary Care*
The safe delivery of healthy twin babies and recovery of their mother in one of the most complex high risk pregnancies recently managed at the hospital represents a remarkable example of multidisciplinary collaboration and clinical excellence.
The woman was 33 weeks pregnant with dichorionic diamniotic twins and was referred after being declined delivery at another tertiary care centre due to the exceptionally high risk nature of her condition. The first time mother had a rare constellation of severe medical factors including severe pre-eclampsia with dangerously high blood pressure, diabetes mellitus requiring insulin therapy, morbid obesity with a body mass index of 67 and weight of 168 kg, compromised cardiac function with a left ventricular ejection fraction (LVEF) of approximately 30–35%, and severe fetal growth restriction in one of the twins. The pregnancy itself was high risk because it was an IVF pregnancy.
Acknowledging the enormity of the challenge, a multidisciplinary team consisting of Dr. Deepika Aggarwal, Chief Gynae Laparoscopy and Robotic Surgery, Dr. Rajesh Misra, Chairperson – Anesthesiology and Dr. Srishti Aggarwal, Consultant Obstetrics & Gynaecology, formulated an all encompassing treatment strategy. Experts in Cardiology, Intensive Care, Paediatrics, Anaesthesia and Surgery worked together to fine-tune every detail of care. The wider multidisciplinary team also included Dr. Jeetendra Sharma and the ICU team, Dr. Hemant Gogia and the Department of Paediatrics, Dr. Sameer Mehrotra and the Cardiology team, and Dr. Mayank Madan and team.
An elective Lower Segment Caesarean Section was successfully performed on 13 April 2026. The procedure, covering anaesthetic induction and surgery, lasted almost two hours, a complex operation that necessitated careful planning because of the serious risks of massive bleeding requiring blood transfusions and possible hysterectomy, heart failure, pulmonary embolism, and other life threatening complications.
