*IVL IntraVascular Lithotripsy for calcific coronary artery lesions *_
The most common cause of angioplasty and stenting failure is secondary to severe coronary artery calcification.

What is it
Intravascular lithotripsy delivers circumferential, unfocused, and pulsatile energy to disrupt calcium within the target lesion safely with > 90% clinical and angiographic success. The purpose is to fracture the calcified plaque to allow proper expansion with a balloon to gain enough lumen diameter to pass and deploy the DES successfully.
What Are the Clinical Implications?
Intravascular lithotripsy can offer a significant improvement in the vessel lumen to facilitate coronary stent delivery and deployments in severely calcified coronary arteries.
Presenting a successful IVL and subsequent PTCA with stenting to calcific 95% LAD lesion in a patient with LV dysfunction .

Presenting a case of elderly individual with calcified LAD block which was successfully treated with IVL and stenting .

Diamondback 360™ Coronary Orbital Atherectomy System (OAS) device (Abott)
A 60 year old male premorbid K/C/O diabetes mellitus and Hypertension old PTCA with stenting to LAD done at bangalore, (Previously stenting was tried at another center in hubli and failed due to dense calcium )and patient came to our centre 🏥HCG SUCHIRAYU hospital Hubli . We used the -Orbital atherectomy device by Abott which is a specialised device to modify and fracture🦴 the calcified tissue and underwent successful stenting to Right coronary artery
Orbital atherectomy has a 1.25 mm diamond coated crown which erodes the calcium into fine particles and clears the hard calcium so the the stent can be placed correctly and apposes the blood vessel wall preventing any future complications .
