32 Y,female...denovo hypertensive...came with decreased responsiveness follow by vomitings..no previous comorbidities...GCS at presentation e3v2m5,PEARL,Rt side grade 2 power...intubated initially managed conservatively for about 10days ,,pt had improved ,to e3vtm6,,last night again had GCS fall to e2vtm5 pearl,rt side paucity...pt as emergency underwent left FTP craniotomy and evacuation of hemtoma ..
Initial ct angio brain done its normal...
Hypertensive bleed ,,is one of the major morbid condition wr pt loose lyf ... immediate action is mandatory to save pt, lyk bp control, antiedema and if meets criteria shud undergo surgical intervention ...today 2nd day of surgery... spontaneous eye opening +,,still M5 score ..hope for recovery

75 yrs male, presented with complaints of headache since 3days,,on and off vomitings,,No major or noticebale trauma history...Right side hemiparesis,,kco HTN ,CAD on tab.ecosprin ,stopped 1month back..GCS E4V4M6,Pearl,RT UL grade 3,RT LL garde 1...CT taken showed Left FTP chronic SDH... Usually in elderly people bleed in the brain can be due to trival trauma to head ,,slow chronic bleed due to shearing force of head and can be due to antiplatelets use,acute bleed can chage to chronic later after 1 r 2months ,some times 6months and present with complaints lyk above...
PATIENT UNDERWENT LEFT PARIETAL SINGLE BURR HOLE CRANIECTOMY AND EVACUATION OF CHRONIC SDH...with one small burr we can evacuate the sdh give proper saline wash .. immediate post op pt extubated, GCS E4V4M6 Pearl,Rt side both UL and LL improved to grade 4..
